The checklist below indicates all the information that needs to be submitted as part of the site plan
application. All required information must be submitted prior to the final approval of any site plan.
The Department of Planning and Development will review the application to ensure that it is complete.
If any portion is not included or complete, the site plan application will not be accepted and returned
to the applicant(s)-

Enclosed is your approved Land Use Permit #855-05469 to cover the reconstruction
and maintenance of one commercial entrance at the referenced location.
Please ensure a copy of the approved permit assembly is kept at the job site whenever
working within the Route 50 right-of-way. I would also like to remind you of the need
to erect and maintain proper traffic control in accordance with our Virginia Work Area
Protection Manual.
For your information, we will need a copy of the recorded joint use agreement before
completion of this project can take place. An example of such is enclosed.
Please give Mr. Dave Heironimus a call at (540) 535-1828 at least 48 hours in advance
of the start of construction to arrange for any necessary inspections.

Dear John:
The Frederick County Department of Planning and Development administratively approved the above -
referenced site plan on August 8, 2000. This site plan, for the construction of a new United States Post
Office in Gore, Virginia, meets all County requirements for development within the RA (Rural Areas)
Zoning District. Specifically, this site plan allows for the construction of a new 3,600-square-foot facility.
The existing entrance to the Gore Post Office will be improved in order to serve the new Post Office.
Adequate parking has been provided in the form of twenty-seven (27) spaces, including two (2) handicap
accessible spaces. The site plan allows for adequate landscaping to mitigate the impact of headlights on
Route 50.
I have provided you with several copies of the approved site plan. Please forward these, approved plans to
the owner and advise the owner to keep an approved copy for their files. Furthermore, the owner should
ensure that an approved copy is maintained on the construction site throughout the development process.
Once site development has been completed, the owner should contact this office to schedule an on -site
inspection.

by Painter-Lcwis, P.I,.C., have been received by this Department. The plans and specifications cousitil w
four sheets titled "United States Post Office, U. S. Route 50, Back Crock Magisncrial District, Gore,
Virginia" and dated 19 May 2000,
A General Permit (VpDFS Permit #VAG8401933) has been issued for the design flow of less than
gpd by the Virginia Department of Environmental Quality (DEQ).
The proposed treatment works consisrs of the installation of a 150 gpd recirculating sandfilter system »• i I
effluent chlorination. The final discharge flows to a drainage awale on the property.
The cvaluxdon of the project design has been confined to technical regrurements and design criteria as
stipulate in the Commonwealth of Virginia Sewe)•uge Regulations (12 VAC 5-580), together with cuncndy
accepted standards of practice and bcstpro tessioualjudgement. It is recomrnended that a water merer be
installed to record water usage to indicate inflow to the sewage tteattnent system.
In accordance with Section 32.1-164 of the C'nde of ruginia, as amendod, this letter constitute permission u

attendance of the contractor, various County agencies and VDOT prior to initiation
of work.
Materials used and method of construction shall apply to current observed VDOT
Road & Bridge Specifications applicable during construction of this development.
• Our review and comments are general in nature. Should conditions in the field exist
such that additional measures are warranted, such measures shall be completed to
the satisfaction of the Department prior to inclusion into the Secondary Road
System.
• All drainage is to be carried within the right-of-way in ditch lines or gutters along the
street to a pipe or drainage easement.
• The contractor shall notify VDOT when work is to begin or cease for any
undetermined length of time. VDOT will also require 48 hours notice for inspections.

State assigned Inspector.

Dear John:
I have had the opportunity to review the above -referenced site plan submitted to our department on June
1, 2000. The following comments should be addressed and included in a revision of this site plan. I have
also marked the enclosed plan for your use.
Please ensure that all adjoining property information is correct. Specifically, please note that
Parcel 28-A-14 is zoned B-2. In addition, please provide the use of all properties located across
the Route 50 right-of-way and eliminate the extra PIN (28-A-63C) on those properties.
2. Please consider connecting the two concrete walkways to facilitate pedestrian movement around
the facility.
3. Please state the name of U.S. Route 50 on the site plan.
4. Please show the location of the proposed electric service and provide a statement ensuring the
underground placement of all new utilities.
5. Please show the location and nature of all fire lanes, hydrants, and other facilities needed to meet
the requirements of the Fire Marshal.

A partial VDOT review has been performed on the preliminary site plan dated May 19, 2000 for
the referenced project. Our comments are in general and follow:
1. The current issue, revised 09/03/98, of the Minimum Standards of Entrances to State
Highways should be the source reference for geometric configuration of proposed access to
USPO site from the VDOT facility.
2. Calculation for provision of right turn taper into the site should be provided for east bound
traffic from Route 50. A copy of HTRIS AADT covering this leg of Route 50 is attached for
your convenience. The calculation should estimate the directional percentage split for PHV
entering the site, to determine some appropriate volume wishing to access site by way of
right turn. Guidelines for Right Turn Treatment (4 Lane Highway) Figure C-1-9 found in the
VDOT Road Design Manual, Volume 1.
3. If the right turn PHV warrants a taper, 150' of taper should be provided. Taper pavement
structure should match existing Route 50 typical (copy attached). The typical Route 50
shoulder should be provided along any taper required. A copy of the portion of the Route 50
plan is also attached for your use.
All the above data is basic information which should be provided on any developmental site plan
proposing to access a VDOT facility.
Once all the above comments have been addressed, please provide VDOT with one set of revised
plans for review.

I have reviewed the sewage treatment portion of the site plan for the post office in Gore, Virginia, and
have the following comments:
1. No specifications or details are provided for the chlorinator. Thirty minutes of effective contact
time must be provided. This may be accomplished separately or by baffling the effluent pump
tank.
2. No details (size; hole spacing) or specifications are provided for the filter underdrains.
3. The effluent line must be shown in a profile view. Details of the outfall structure should be
provided. A sign should be posted to warn the effluent is treated sewage.
4. A maintenance contract will be required for this type of system.

following comments:
• Final plans and specifications for the proposed discharging sewage disposal system
are to be forwarded to the Office of Water Programs according to John Lewis.
• An adequate easement in perpetuity for all portions of the discharging sewage
disposal system to be installed on adjacent properties and for the required 500 feet of
discharge channel must be shown on the plans. This easement must be properly
recorded in the land records of the Frederick County Courthouse.
• A proposed well is not shown on the plans. An application for a well construction
permit must be submitted to this office. If the well is proposed for public use or the
proposal for the well use meets the criteria of a public water supply, the application
should be submitted to the Office of Water Programs.
• The existing well serving the existing post office is approximately 35 feet from the
proposed discharge channel. The setback distance required as per Table 3.1 of the
Alternative Discharge Regulations is 100 feet minimum.
