All work and materials shall conform to the current standards of the County of Frederick, Virginia, the Virginia Department of Transportation (V.D.O.T.), the Virginia Uniform Statewide Building Code, and the rules and regulations of the Americans with Disabilities Act, where applicable. 2. Erosion and sediment control shall conform to the standards and specification of the Commonwealth of Virginia and the County of Frederick, Virginia, where applicable. 3. It shall be the responsibility of the contractor to notify "Miss Utility" at 1-800-552-7001 at least two (2) working days prior to commencement of any land disturbing activity. 4. All existing utilities have been shown based upon the best available information. However, there may be existing utilities which are not shown and should be located. Therefore, it shall be the responsibility of the contractor to verify the location of all existing utilities prior to construction. Verification shall be done by way of a field survey or excavated test pits. Any discrepancies between these plans and the actual field conditions shall be reported immediately to the engineer, the owner, and/or the County of Frederick, Virginia. 5. All proposed utilities shall be installed underground. 6. The Contractor shall examine the site and become familiar with the existing conditions prior to construction. Failure to do so does not relieve the Contractor of this responsibility or permit the issuance of Change Orders for unforeseen work. 7. It shall be the responsibility of the contractor to obtain all permits necessary for construction prior to commencement of work. An approved set of construction documents shall be present on the site at all times. 8. Topographic information was derived based upon U.S.G.S. datum by an field survey performed by the survey division of PainterLewis, P.L.C. and established at a one (1') foot contour interval. 9. No Geo-technical report has been furnished for this project. 10. All radii designations indicate face of curb, edge of pavement, or edge of stone pavement, where applicable. 11. The contractor shall coordinate the relocation of and the connection to all existing utilities with the appropriate utility company, if applicable. All erosion and sediment control measures shall be installed prior to any land disturbing activity. All work shall be done in accordance with the current edition of the Commonwealth of Virginia Erosion and Sediment Control Handbook. 2. All work shall be confined to the designated limits of construction. For this project, the limits of construction are defined as the property boundaries. 3. No disturbed areas shall be denuded for more than thirty (30) days. The contractor shall stabilize all denuded areas within seven (7) days after the end of construction. 4. The contractor shall be responsible for the installation and maintenance of all erosion and sediment control measures. All measures shall be inspected daily and after each significant rainfall by the site superintendent or his representative. Any damaged structures shall be repaired or replaced by the end ..of work that day. 5. Upon completion of construction, all permanent erosion and sediment control measures shall be installed. After stabilization, the temporary erosion and sediment control devices shall be removed as approved by the local inspection authority. All vegetative cover shall be checked regularly and any damaged areas shall be repaired, fertilized, replanted, and mulched as necessary. Vegetative cover shall be applied to all denuded areas and shall conform to the following specification: Seedbed Preparation: A. Scarify the top 1"-2" of topsoil after final grades have been achieved. B. Add 3 tons per acre of pulverized agricultural lime (140 lb/1000 sf). Seed Type: Rate: Mixture: Reliant Hard Fescue 130 Ib/Ac. 65% Kentucky Bluegrass 40 Ib/Ac. 20% Perennial Rye grass 30 Ib/Ac. 15% See specifications from supplier for complete information. EXCAVATION AND GRADING: 1. Test pits shall be excavated to determine the depths and locations of all of the existing utilities. If conflicts exist, then the existing utilities shall be lowered or moved to promote the construction of the proposed development. The project engineer, owner, or his representative, and the appropriate utility company shall be notified immediately if a conflict exists. 2. All deleterious materials, demolished debris, and all vegetation that has been cleared and grubbed shall be completely removed, including the root mat, and shall be removed and properly disposed of off -site. 3. All topsoil shall be excavated to its full depth and stockpiled on -site in the area indicated on the plans or as directed by the owner's representative for future finish grading. 4. All borrow fill material, if needed, shall be obtained by the contractor and shall be approved by a certified Geo-technical engineer. All fill material shall be placed in lifts of 8" or less and shall be compacted to 95% of ASTM D-698 maximum dry density, or as directed by the Geo-technical engineer. Fill material may be obtained from the site or as directed by the owner. All compaction testing and the appropriate reports shall be performed for fill placed under all public utilities. The compaction test reports shall be submitted to the F.C.S.A. Department. 5. No blasting shall be permitted within 25 feet of existing utilities or structures without permission of the owner and or the utility companies. If applicable, blasting operations shall be performed at a minimum of 50 feet beyond the end of any terminal utility line. Pre -blast surveys shall be performed of the surrounding structures prior to commencement of any drilling or blasting operations. 6. All slopes, lawn areas, planters, and berms shall be finished with a minimum of 6" of topsoil. All areas shall be graded and raked, prior to application of permanent vegetation cover. 7. All disturbed lawn areas shall be seeded, fertilized, and mulched, as needed. FREDERICK COUNTY SANITATION AUTHORITY NOTES 1. The Frederick County Sanitation Authority's (FCSA or Authority) Water and Sewer Standards and Specifications are available at www.fcsa-water.com. 2. The water and sanitary sewer portion" of these plans are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrange for inspection by, the Authority. 5. When an existing water main is tapped for the installation of a new line: a) The Authority shall furnish all labor and material. b) The developer/contractor must: 1. make application and pay deposit 2. mark the tap location. 6. The contractor shall tie-in a new sewer lines to an existing manhole by core drilling the manhole. 7. Exact location of water and sewer services on new lines are to be coordinated with the Authority's Inspector. 8. The Authority's maintenance department all furnish and install all water meters through 2" in size. It is the developer's/contractor's responsibility to have the meter box assembly installed correctly. Before a permanent meter is installed: a) The meter box, with its frame and cover, must be properly aligned with the coppersetter. b) The frame and cover shall be set to final grade. c) The distance between the top of the cover and the coppersetter shall to be between 18 and 21 inches. d) All components of the meter box assemble shall be in proper working order. 9. For water and sewer services that connect to existing lines: a) The location of the service lateral is to be coordinated with the Authority's Engineering Assistant. b) An Application for Service must be made and fees paid before work will begin. c) The Authority shall furnish and install all 5/8", 3/4" and 1" water services. d) For all 1 1 /2"and 2" water services, the Authority shall make the tap and bring the piping to the property line. The contractor shall furnish and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals. 10. Any water service line must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet ASSE standard No. 1015 or 1013. 11. All fire lines shall have a fire service meter assembly and a backflow prevention assembly (double detector check valve or RPZ, as required). The assembly shall meet ASSE standard No. 1048 or 1047. Radio read remotes are required. STORM SEWER NOTES: 1. A minimum clearance of 18" shall be maintained between the water and sewer mains at all crossings. If 18" of separation cannot be maintained, then concrete encasements and cradles shall be installed as indicated on the plans or as directed by the FCSA. 2. All storm sewer construction shall conform to the standards and specifications of V.D.O.T. All piping shall be as designated on the construction documents to the slopes as indicated on the plans. 3. All proposed storm sewer structures shall meet V.D.O.T. specifications. Cast -in -place flow lines (IS-1 Typ.) shall be installed in all structures to facilitate flow through the system. PAVING AND CONCRETE NOTES: 1. Pavement section: All paved areas shall receive the minimum pavement sections, as detailed on the construction plans. All courses shall be properly finished and compacted. Tie-in joints to the existing pavement shall be saw cut to ensure that the proper connection is obtained. 2. All concrete for this project shall be class A concrete with a minimum compressive strength of 3500 psi. 3. The concrete curb shall be a V.D.O.T. standard CG-6 curb and gutter or reversed pitch CG-6. If applicable, the reversed pitch shall be a minimum of 1.04% or match the pavement cross slope, whichever is greater. 4. All concrete walks shall be 4" in depth and shall be reinforced as indicated on the plans. MISCELLANEOUS: 1. Fire lanes shall be delineated as directed by the County of Frederick, Virginia Fire Department. 2. Building shall comply with The Virginia Uniform Statewide Building Code section 309, use group M (Mercantile), and section 311 use group S (Storage) of the International Building Code/2000. Other code that apply are ICC/ANS A117.1-98. Accessible and Usable Buildings and Facilities. 3. All required man egress door and 50% of the entrances shall comply with accessibility under chapter 11 of the IBC and ICC/ANSI A117.1-98. The door locations is mot indicated on the plan at this time and shall be provided with an accessible route. 4. All structural plans submitted for permit application shall be sealed by a VA Design Professional. Please note the requirements in Chapter 17 of the IBC for special inspections for this type of structure. EROSION AND SEDIMENT CONTROL MEASURES 3.05 Silt fence will be installed in selected locations downstream from the construction area as a first measure of construction. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shall be made immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should be removed after each storm event. They must be removed when deposits reach approximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.08 Silt fence will be installed as culvert inlet protection at the upstream ends of culverts. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shall be made immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should be removed after each storm event. They must be removed when deposits reach approximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.30 Replacement of topsoil will generally occur throughout the work site where vegetation is to be reestablished. Topsoil will be uniformly spread and rolled to a minimum depth of 6". Topsoil shall not be placed while in a frozen of wet condition. 3.31 Temporary seeding in accordance with the E & S Std & Spec 3.31 will be applied to the topsoil stock pile and all areas which will not be brought to final grade within 30 days. Embankment or excavated slopes denuded for a period of greater than 30 days shall be temporarily seeded and mulched. To coincide with the anticipated construction dates, the seeding will consist of 50 pounds per acre of German Millet. All temporary seeding areas will be mulched in accordance with the schedule included herein. 3.32 All disturbed areas will be stabilized by permanent seeding in accordance with the schedule included herein. Permanent stabilization shall be applied to areas that are to be left dormant for more than one year. During construction of the project, soil stockpiles shall be stabilized or protected with sediment trapping measures. The contractor is responsible for the temporary protection and permanent stabilization of all soil stockpiles on site as well as soil intentionally transported from the project site. All erosion and sediment control measures shall be installed prior to any land disturbing activity. All work shall be done in accordance with the current edition of the Commonwealth of Virginia Erosion and Sediment Control Handbook. 2. All work shall be confined to the designated limits of construction. For this project, the limits of construction are defined as the property boundaries. 3. No disturbed areas shall be denuded for more than thirty (30) days. The contractor shall stabilize all denuded areas within seven (7) days after the end of construction. 4. The contractor shall be responsible for the installation and maintenance of all erosion and sediment control measures. All measures shall be inspected daily and after each significant rainfall by the site superintendent or his representative. Any damaged structures shall be repaired or replaced by the end of work that day. 5. Upon completion of construction, all permanent erosion and sediment control measures shall be installed. After stabilization, the temporary erosion and sediment control devices shall be removed as approved by the local inspection authority. All vegetative cover shall be checked regularly and any damaged areas shall be repaired, fertilized, replanted, and mulched as necessary. Vegetative cover shall be applied to all denuded areas and shall conform to the following specification: Seedbed Preparation: A. Scarify the top 1"-2" of topsoil after final grades have been achieved. B. Add 3 tons per acre of pulverized agricultural lime (140 lb/1000 sf). EXCAVATION AND GRADING: 1. Test pits shall be excavated to determine the depths and locations of all of the existing utilities. If conflicts exist, then the existing utilities shall be lowered or moved to promote the construction of the proposed development. The project engineer, owner, or his representative, and the appropriate utility company shall be notified immediately if a conflict exists. 2. All deleterious materials, demolished debris, and all vegetation that has been cleared and grubbed shall be completely removed, including the root mat, and shall be removed and properly disposed of off -site. 3. All topsoil shall be excavated to its full depth and stockpiled on -site in the area indicated on the plans or as directed by the owner's representative for future finish grading. 4. All borrow fill material, if needed, shall be obtained by the contractor and shall be approved by a certified Geo-technical engineer. All fill material shall be placed in lifts of 8" or less and shall be compacted to 95% of ASTM D-698 maximum dry density, or as directed by the Geo-technical engineer. Fill material may be obtained from the site or as directed by the owner. All compaction testing and the appropriate reports shall be performed for fill placed under all public utilities. The compaction test reports shall be submitted to the F.C.S.A. Department. 5. No blasting shall be permitted within 25 feet of existing utilities or structures without permission of the owner and or the utility companies. If applicable, blasting operations shall be performed at a minimum of 50 feet beyond the end of any terminal utility line. Pre -blast surveys shall be performed of the surrounding structures prior to commencement of any drilling or blasting operations. 6. All slopes, lawn areas, planters, and berms shall be finished with a minimum of 6" of topsoil. All areas shall be graded and raked, prior to application of permanent vegetation cover. 7. All disturbed lawn areas shall be seeded, fertilized, and mulched, as needed. FREDERICK COUNTY SANITATION AUTHORITY NOTES 1. The Frederick County Sanitation Authority's (FCSA or Authority) Water and Sewer Standards and Specifications are available at www.fcsa-water.com. 2. The water and sanitary sewer portion of these plans are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrange for inspection by, the Authority. 5. When an existing water main is tapped for the installation of a new line: a) The Authority shall furnish all labor and material. b) The developer/contractor must: 1. make application and pay deposit 2. mark the tap location. 6. The contractor shall tie-in a new sewer lines to an existing manhole by core drilling the manhole. 7. Exact location of water and sewer services on new lines are to be coordinated with the Authority's Inspector 8. The Authority's maintenance department all furnish and install all water meters through 2" in size. It is the developer's/contractor's responsibility to have the meter box assembly installed correctly. Before a permanent meter is installed: a) The meter box, with its frame and cover, must be properly aligned with the coppersetter. b) The frame and cover shall be set to final grade. c) The distance between the top of the cover and the coppersetter shall to be between 18 and 21 inches. d) All components of the meter box assemble shall be in proper working order. 9. For water and sewer services that connect to existing lines: a) The location of the service lateral is to be coordinated with the Authority's Engineering Assistant. b) An Application for Service must be made and fees paid before work will begin. c) The Authority shall furnish and install all 5/8", 3/4" and 1" water services. d) For all 1 1 /2"and2" water services, the Authority shall make the top and bring the piping to the property line. The contractor shall furnish and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals. 10. Any water service line must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet ASSE standard No. 1015 or 1013. 11. All fire lines shall have a fire service meter assembly and a backflow prevention assembly (double detector check valve or RPZ, as required). The assembly shall meet ASSE standard No. 1048 or 1047. Radio read remotes are required. STORM SEWER NOTES: 1. A minimum clearance of 18" shall be maintained between the water and sewer mains at all crossings. If 18" of separation cannot be maintained, then concrete encasements and cradles shall be installed as indicated on the plans or as directed by the FCSA. 2. All storm sewer construction shall conform to the standards and specifications of V.D.O.T. All piping shall be as designated on the construction documents to the slopes as indicated on the plans. 3. All proposed storm sewer structures shall meet V.D.O.T. specifications. Cast -in -place flow lines (IS-1 Typ.) shall be installed in all structures to facilitate flow through the system. PAVING AND CONCRETE NOTES: 1. Pavement section: All paved areas shall receive the minimum pavement sections, as detailed on the construction plans. All courses shall be properly finished and compacted. Tie-in joints to the existing pavement shall be saw cut to ensure that the proper connection is obtained. 2. All concrete for this project shall be class A concrete with a minimum compressive strength of 3500 psi. 3. The concrete curb shall be a V.D.O.T. standard CG-6 curb and gutter or reversed pitch CG-6. If applicable, the reversed pitch shall be a minimum of 1.04% or match the pavement cross slope, whichever is greater. 4. All concrete walks shall be 4" in depth and shall be reinforced as indicated on the plans. MISCELLANEOUS: 1. Fire lanes shall be delineated as directed by the County of Frederick, Virginia Fire Department. 2. Building shall comply with The Virginia Uniform Statewide Building Code section 309, use group M (Mercantile), and section 311 use group S (Storage) of the International Building Code/2000. Other code that apply are ICC/ANS A117.1-98. Accessible and Usable Buildings and Facilities. 3. All required man egress door and 50% of the entrances shall comply with accessibility under chapter 11 of the IBC and ICC/ANSI A117.1-98. The door locations is mot indicated on the plan at this time and shall be provided with an accessible route. 4. All structural plans submitted for permit application shall be sealed by a VA Design Professional. Please note the requirements in Chapter 17 of the IBC for special inspections for this type of structure. EROSION AND SEDIMENT CONTROL MEASURES 3.05 Silt fence will be installed [in selected locations downstream from the construction area as a first measure of construction. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shall be made immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should be removed after each storm event. They must be removed when deposits reach approximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.08 Silt fence will be installed as culvert inlet protection at the upstream ends of culverts. Silt fences shall be inspected after each rainfall and at least daily during prolonged rainfall. Any required repairs shall be made immediately. Damaged, decomposed or otherwise ineffective silt fence shall be replaced. Sediment deposits should be removed after each storm event. They must be removed when deposits reach approximately one-half the height of the barrier. Any sediment deposits remaining in place after the silt fence is no longer needed shall be dressed to conform with the existing grade and stabilized. 3.30 Replacement of topsoil will generally occur throughout the work site where vegetation is to be reestablished. Topsoil will be uniformly spread and rolled to a minimum depth of 6". Topsoil shall not be placed while in a frozen of wet condition. 3.31 Temporary seeding in accordance with the E & S Std & Spec 3.31 will be applied to the topsoil stock pile and all areas which will not be brought to final grade within 30 days.
