CONSTRUCTION NOTES 70 W re GENERAL$T7M A ��'l , (A7L1 1. The contractor shall furnish all materials, labor and equipment and perform all work including restoration for the completed installation of the utility systems and all other improvements shown or implied as necessary for the completed facility ready for use. Unless otherwise rooted, specifications for all work to be in accordance with applicable Standards and Construction Specifications of The Virginia Department of Transportation, Virginia Department of Health, Frederick County Sanitation Auuthontyy and the contract documents. 2. The engineer has attempted to locate existing s3ubsurface utilities; however, such may exist that are not shown. The contractor shall exercise care in this work so ms to avoid damage to any utilities. Any damage shall be the contractor's responsibility. The contractor shall notiffy all utility companies 72 hours prior to construction. Call "Miss Utility" 1-800-552-7001. 3. The contractor shall be responsible for all consitruction stakeout. All dimensions, elevations and locations shall be verified in the field by each contractor prior to !beginning the work. The Engineer will approve all cut sheets. 4. Any unusual subsurface conditions encountered idunng the course of the work shall be immediately brought to the attention of the Engineer and/or the Owner. 5. Contractor to obtain, at his expense, all permits required of this work and shall faithfully adhere to the permit conditions imposed. 6. All grading for improvements to be confined to the project area. Any work performed offsite is the Contractor's responsibility. 7. Cutsheets shall be submitted to the Engineer foir approval prior to ordering structures and start of construction. All cutsheets should show existing ground elevations at centerline of utility, structure or road, and offset hub elevation. FREORRICK COUNTY SANITATION AUTHiORITY 1. The Authority's Water and Sewer Standards and Specifications are available at www.fcsa-water.com . 2. The water and sanitary sewer portion of these plains are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrange for inspection by, the Authority. 5. When an existing water main is tapped for the imstallation of a new line: a) The Authority shall furnish all labor and (material. b) The developer/contractor must: 1. make application and pay deposit 2. mark the tap location 3. coordinate installation with the engirneering-assistant. 6. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's Inspector. 7. The Authority's maintenance department shall furni-sh and install all water meters through 2" in size. It is the developer's/contractor's responsibility to thave the meter box assembly installed correctly. Before a permanent meter is installed: a) The meter box, with its frame and cover, murst be properly aligned with the coppersetter. b) The frame and cover shall be set to final grade. c) The distance between the top of the cover and the coppersetter shall be between 18 and 21 inches. d) All components of the meter box assembly shhall be in proper working order. 8. For water and sewer services that connect to existing lines: a) The location of the service lateral is to be c(oordinated with the Authority's Engineering Assistant. b An Application for Service must be made and: fees paid before work will begin. c The Authority shall furnish and install all 5/8"', 3/4" and 1" water services. d) For all 1 V and 2" water services, the Authtority shall make the tap and bring the piping to the property line. The contractor shall furnisth and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals. 9. Any water service line must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet ASSE standard) No. 1015 or 1013. 10. All fire lines shall have a fire service meter assermbly and a backflow prevention assembly (detector double check valve or RPZ, as required). The assembly shall meet ASSE standard No. 1048 or 1047. Radio read remotes are required. VIRGINIA DEPARTMENT OF TRANSPORTATION V1. All work on this project shall conform to the current editions of and latest revisions to the Virginia Department of Transportation (VDOT) Road and Bridge Specifications and Standards, the Virginia Erosion and Sediment Control Regullations, and any other applicable state, federal or local regulations. In case of a discrepancy or conflict between the Standards or Specifications and Regulations, the most stringent shall govern V2. All construction shall comp) with the latest U.S. Department of Labor, Occupational Safety and Health Administration (OSHA, and Virginia Occupational Safety & Health (VOSH) Rules and Regulations. V3. When working within VDOT right-of-way, all trofffic control, wheather permanent or temporary, shall be in accordance with the current edition of VDOT's Work Area Protection Manual. Furthermore, all traffic control flaggers must be certified in accordance with Section 104.04(c) of the VDOT Road and Bridge Specifications. V4 The developer shall be responsible for relocating, at his expense, any and all utilities, including traffic signal poles, junction boxes, controllers, ietc., owned by VDOT or private / public utility companies. It is the sole responsibility of the dleveloper to locate and identify utility facilities or items that may be in conflict with the proposecd construction activity. VDOT approval of these plans does not indemnify the developer from thiis responsibility. V5. Design features relating to field construction, regulations, and control or safety of traffic may be subject to change as deemed necessary by VDOT. Any additional expense incurred as a result of any field revision shall be the responsibility of the developer. V6. Prior to initiation of work, the contractor shall be responsible for acquiring all necessary VDOT land use permits for any work within VDOT righit-of-way. V7. If required by the local VDOT Residency Office, a pre -construction conference shall be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. V8. The contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT requires and shall receive 48 hours advance notice prior to any required or requested inspection. V9. The contractor shall be responsible for maintaining adequate access to the project from the adjacent public roadway via a construction entrance that is constructed and maintained in accordance with Section 3.02 of the Virginia Eroosion and Sediment Control Handbook. Furthermore, access to other properties offectedf by this project shall be maintained through construction. The Developer shall have, within thie limits of the project, an employee certified by the Virginia Department of Conservation and Reccreation (VDCR) in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures on a continuous basis for proper installation and operation. De nciencies shall be promptly rectified. V10. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. V11. All water and sewer lines within existing or proposed VDOT right-of-way shall have a minimum thirty-six (36) inches cover and where possible be installed under roadway drainage facilities at conflict points. V12. Any unusual subsurface conditions (e.g., unsuitalble soils, springs, sinkholes, voids, caves, etc.) encountered during the course of construction shall be immediately brought to the attention of the engineer and VDOT. Work shall cease in that vicinity until an adequate design can be; determined by the engineer and approved by VDOT. V13. All fill areas, borrow material and undercut areas shall be inspected and approved by a VDOT representative prior to placement antd fill. Where CBR testing is required, a VDOT representative shall be present, to insure the sample obtaines is representative of the location. When soil samplers are submitted to private laboratories for testing, the samples shall be clearly identified and labeled as belonging to a project to be accepted by VDOT and that testing shall be performed in accordance with all applicable VDOTT standards and procedures. Tim Hockman All roadway fill, base, subgrade material, and backfill in utility/storm sewer trenches shall be compacted in six (6) inch lifts to 95% of theoretical maximum density as determined by AASHTO T-99 Method A, within plus or minus 2% of optimum moisture for the full width of any dedicated street right-of-way. At the direction of VDOT, density tests shall be performed by a qualified independent ogenQ1 in accordance with VDOT Road and Bridge Specifications. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise VDOT Standard CD and UD underdrains shall be installed where indicated on these plans and/or as specified by VDOT. The installation of any entrances and mailboxes within any dedicated street right- of-way shall meet VDOT minimum design standards and is the responsibility of the developer. Prior to VDOT acceptance of any streets, all required street signage and/or pavement markings shall be installed by the developer or, at VDOT's discretion, by VDOT on an account receivable basis following the Manual On Uniform Traffic Control Devices. The developer shall provide the VDOT Residency Office with a list of all material sources prior to the start of construction. Copies of all invoices for materials utilized within any dedicated street right-of-way must be provided to the local VDOT Residency Office prior to acceptance of the work. Unit and total prices may be obscured. Aggregate base and subbase materials shall be placed on subgrade by means of a mechanical spreader. Density will be determined using the density control strip in accordance with Section 304 of the VDOT Road and Bridge Specifications and VTM-10. A certified compaction technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. In addition to checking stone depths, a VDOT representative shall be notified and the opportunity to be present during the construction and testing of the density control strip. Asphalt concrete pavements shall be placed in accordance with Section 315 of the VDOT Road and Bridge Specifications. Density shall be determined using the density control strip as specified in Section 315 and VTM-76. A certified compaction technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. A VDOT representative shall be notified and given the opportunity to be present during the construction and testing of the control strip. In accordance with Section 302.03, the foundations for pipe culverts thirty-six- (36) inches and larger shall be explored below the bottom of the excavation to determine the type and condition of the foundation. The contractor shall report findings of foundation exploration to the engineer and VDOT for approval prior to placing pipe. Foundation designs shall comply with VDOT Road and Bridge Standard PB-1. Where soft, yielding, or otherwise unsuitable foundation is encountered, the foundation design and/or need for foundation stabilization shall be determined by the engineer and approved by VDOT. Approval of these plans shall expire three (3) years from the date of the approval letter. V23. VDOT Standard CG-12 Curb Ramps shall be installed where indicated on these plans and/or as specified by VDOT. V24. VDOT Standard Guardrail shall be installed where warranted and/or as proposed on these plans in accordance with VDOT?s installation criteria. Temporary and permanent erosion control structures and methods to be in accordance with the Virginia Erosion Control Guidelines and local ordinances and to be utilized during the construction period. 5. All erosion control structures shown or as required during construction shall be continuously maintained throughout the period in which areas are denuded. Areas shown to be surfaced are to have stone base installed at the earliest possible date after establishing grade. No area is to remain denuded for more than 7 calendar days when construction is not in progress. 6. Contractor to clean up and restore disturbed areas immediately upon completion of pipe -laying in the affected area. 7. All unpaved areas of the site which are disturbed by this construction shall be provided with 6" topsoil, fertilized, seeded, compacted and mulched. Seed bed to be watered and cared for as necessary to provide a good lawn quality grass cover. Areas not graded shall be disced, leveled and seeded to achieve lawn conditions. FIRE & RESCUE 1. Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited. 2. Access for emergency vehicles must be maintained at all times during construction. Temporary street signs and addresses must be posted as construction begins. Parked vehicles and staging of construction materials are prohibited within 20' of fire hydrants. Fire Hydrants are not to be obstructed at any time. Fire lanes are to be a minimum width of 18 feet. 3. Fire lanes shall be designated in either of the following ways: A. Double yellow, six (6) inch lines with approximately three (3) inches between lines shall be used to mark paved areas. B. Painting of curbing yellow. C. The Authority's Water and Sewer Standards amd Specifications are available at www.fcsa-water.com . 2. The water and sanitary sewer portion of thesie plans are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrainge for inspection by, the Authority. 5. When an existing water main is tapped for tthe installation of a new line: a) The Authority shall furnish all labor land material. b) The developer/contractor must: 1. make application and pay deposit 2. mark the tap location 3. coordinate installation with the (engineering -assistant. 6. Exact locations of water and sewer services con new lines are to be coordinated with the Authority's Inspector. 7. The Authority's maintenance department shall furnish and install all water meters through 2" in size. It is the developer's/contractor's responsibiiityr to have the meter box assembly installed correctly. Before a permanent meter is installed: a The meter box, with its frame and cover,, must be properly aligned with the coppersetter. b The frame and cover shall be set to final grade. c The distance between the top of the covfer and the coppersetter shall be between 18 and 21 inches. d All components of the meter box assembly shall be in proper working order. 8. For water and sewer services that connect to existing lines: a) The location of the service lateral is to Ibe coordinated with the Authority's Engineering Assistant. b An Application for Service must be made and fees paid before work will begin. c The Authority shall furnish and install all 5/8", 3/4" and 1" water services. d For all 1 X" and 2" water services, the Authority shall make the tap and bring the piping to the property line. The contractor shall ffumish and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals;. 9. Any water service line must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet ASSE stomdard No. 1015 or 1013. 10. All fire lines shall have a fire service meter mssembly and a backflow prevention assembly (detector double check valve or RPZ, as required). Th(e assembly shall meet ASSE standard No. 1048 or 1047. Radio read remotes are required. VIRGINIA DEPARTMENT OF TRANSPORTATION V1. All work on this project shall conform to the current editions of and latest revisions to the Virginia Department of Transportation (VDOTF) Road and Bridge Specifications and Standards, the Virginia Erosion and Sediment Control (Regulations, and any other applicable state, federal or local regulations. In case of a discrepancy or conflict between the Standards or Specifications and Regulations, the most stringent shall govern V2. All construction shall compl with the latest U.S. Department of Labor, Occupational Safety and Health Administration (OSHA, and Virginia Occupational Safety &Health (NOSH) Rules and Regulations. V3. When working within VDOT right-of-way, all traffic control, wheather permanent or temporary, shall be in accordance with the current edition of VDOT's Work Area Protection Manual. Furthermore, all traffic control flaggers must be certified in accordance with Section 104.04(c) of the VDOT Road and Bridge Specifications. V4 The developer shall be responsible for relocating, at his expense, any and all utilities, including traffic signal poles, junction boxes, controlliers, etc., owned by VDOT or private / public utility companies. It is the sole responsibility of tthe developer to locate and identify utility facilities or items that may be in conflict with the proiposed construction activity. VDOT approval of these plans does not indemnify the developer frolm this responsibility. V5. Design features relating to field constructioln, regulations, and control or safety of traffic may be subject to change as deemed necessary by/ VDOT. Any additional expense incurred as a result of any field revision shall be the responsibility, of the developer. V6. Prior to initiation of work, the contractor shall be responsible for acquiring all necessary VDOT land use permits for any work within VDOT right-of-way. V7. If required by the local VDOT Residency Offfice, a pre -construction conference shall be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT prior to initiation of work. V8. The contractor shall notify the local VDOT (Residency Office when work is to begin or cease for any undetermined length of time. VDOT reqluires and shall receive 48 hours advance notice prior to any required or requested inspection. V9. The contractor shall be responsible for maiintaining adequate access to the project from the adjacent public roadway via a construction entrance that is constructed and maintained in accordance with Section 3.02 of the Virginiia Erosion and Sediment Control Handbook. Furthermore, access to other properties offfected by this project shall be maintained through construction. The Developer shall have, withiin the limits of the project, an employee certified by the Virginia Department of Conservation ands Recreation (VDCR) in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures on a continuous basis for proper installation and operation. Deficiencies shall be promptly rectified. V10. Contractor shall ensure adequate drainage its achieved and maintained on the site during and at the end of construction. V11. All water and sewer lines within existing or proposed VDOT right-of-way shall have a minimum thirty-six (36) inches cover and where possible be installed under roadway drainage facilities at conflict points. V12. Any unusual subsurface conditions (e.g., unsuitable soils, springs, sinkholes, voids, caves, etc.) encountered during the (course of construction shall be immediately brought to the attention of the; engineer and VDOT. Work shall cease in that vicinity until an adequate design cam be determined by the engineer and approved by VDOT. V13. All fill areas, borrow material and undercut areas shall be inspected and approved by a VDOT representative prior to placement and fill. Where CBR testing is required, a VDOT representative shall be present to insure the sample obtaines is representative of the location. When soil salmples are submitted to private laboratories for testing, the samples shall be clearly identified and labeled as belonging to a project to be accepted by WDOT and that testing shall be performed in accordance with all applicable VDOT standards and procedures. Tim Hackman All roadway fill, base, subgrade material, and backfill in utility/storm sewer trenches shall be compacted in six (6) inch lifts to 95% of theoretical maximum density as determined by AASHTO T-99 Method A, within plus or minus 2% of optimum moisture for the full width of any dedicated street right-of-way. At the direction of VDOT, density tests shall be performed by a qualified independent agency in accordance with VDOT Road and Bridge Specifications. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise VDOT Standard CD and UD underdrains shall be installed where indicated on these plans and/or as specified by VDOT. The installation of any entrances and mailboxes within any dedicated street right- of-way shall meet VDOT minimum design standards and is the responsibility of the developer. Prior to VDOT acceptance of any streets, all required street signage and/or pavement markings shall be installed by the developer or, at VDOT's discretion„ by VDOT on an account receivable basis following the Manual On Uniform Traffic Control Devices. The developer shall provide the VDOT Residency Office with a list of all material sources prior to the start of construction. Copies of all invoices for materials utilized within any dedicated street right-of-way must be provided to the local VDOT Residency Office prior to acceptance of the work. Unit and total prices may be obscured. Aggregate base and subbase materials shall be placed on subgrade by means of a mechanical spreader. Density will be determined using the density control strip in accordance with Section 304 of the VDOT Road and Bridge Specifications and VTM-10. A certified compaction technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. In addition to checking stone depths, a VDOT representative shall be notified and the opportunity to be present during the construction and testing of the density control strip. Asphalt concrete pavements shall be placed in accordance with Section 315 of the VDOT Road and Bridge Specifications. Density shall be determined using the density control strip as specified in Section 315 and VTM-76. A certified compaction technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwise. A VDOT representative shall be notified and given the opportunity to be present during the construction and testing of the control strip. In accordance with Section 302.03, the foundations for pipe culverts thirty-six- (36) inches and larger shall be explored below the bottom of the excavation to determine the type and condition of the foundation. The contractor shall report findings of foundation exploration to the engineer and VDOT for approval prior to placing pipe. Foundation designs shall comply with VDOT Road and Bridge Standard P13-1. Where soft, yielding, or otherwise unsuitable foundation is encountered, the foundation design and/or need for foundation stabilization shall be determined by the engineer and approved by VDOT. Approval of these plans shall expire three (3) years from the date of the approval letter. V23. VDOT Standard CG-12 Curb Ramps shall be installed where indicated on these plans and/or as specified by VDOT. V24. VDOT Standard Guardrail shall be installed where warranted and/or as proposed on these plans in accordance with VDOT?s installation criteria. Prior to initiating any land disturbing activity, it will be necessary to schedule an on -site pre -construction meeting with public works staff to discuss the approved erosion and sediment control plan. No pre-construc+ion meetings will be scheduled prior to issuance of the land disturbance permit for the subject project. 2. Contractor to contact the public works department to schedule the required erosion fnd sediment control measures inspections as indicated by the program administrator. If the erosion and sediment control measurers helve been installed in accordance with the approved plan, the Department of Public Works will grant approval to initiate land disturbing activities. 3. A letter from the engineer -of -record shall be included with the as -built which states that the subdivision has been constructed in accordance with the approved plan. c). Individual subdivision lot grading plans shall be submitted by the builder at the time of the building permit application for each lot proposed for construction. 4. Temporary and permanent erosion control structures and methods to be in accordance with the Virginia Erosion Control Guidelines and local ordinances and to be utilized during the construction period. 5. All erosion control structures shown or as required during construction shall be continuously maintained throughout the period in which areas are denuded. Areas shown to be surfaced are to have stone base installed at the earliest possible date after establishing grade. No area is to remain denuded for more than 7 calendar days when construction is not in progress. 6. Contractor to clean up and restore disturbed areas immediately upon completion of pipe -laying in the affected area. 7. All unpaved areas of the site which are disturbed by this construction shall be provided with 6" topsoil, fertilized, seeded, compacted and mulched. Seed bed to be watered and cared for as necessary to provide a good lawn quality grass cover. Areas not graded shall be disced, leveled and seeded to achieve lawn conditions. FIRE & RESCUE 1. Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited. 2. Access for emergency vehicles must be maintained at all times during construction. Temporary street signs and addresses must be posted as construction begins. Parked vehicles and staging of construction materials are prohibited within 20' of fire hydrants. Fire Hydrants are not to be obstructed at any time. Fire lanes are to be a minimum width of 18 feet. 3. Fire lanes shall be designated in either of the following ways: A. Double yellow, six (6) inch lines with approximately three (3) inches between lines shall be used to mark paved areas. B. Painting of curbing yellow. C. By painting of yellow hatch marks of six (6) inches in width with no more than ten(10) inches between hatches on the entire area being designated as a fire lane. 4. Signs shall be twelve (12) by eighteen (18) inches with a white background, with red two (2) inch block letters with the wording, "NO PARKING FIRE LANE Authority of Fire Chief". Any other color combination or wording needs to be approved by the Fire Chief. 5. Signs shall be posted on all comers of the fire lane, and then no further than fifty (50) feet apart along the remaining length of fire lane area. Signs shall be a minimum height of seven (7) feet to the bottom of the sign and a maximum height to the top of sign shall be nine (9) feet. Any other height needs to be approved by the Fire Chief. 6. Any existing markings which will be within the fire lane area after their establishment shall be removed so as not to create confusion. 7. The fire lane shall be maintained on a regular basis to insure these lanes can be easily recognized. Maintenance shall include repainting on lines or curbing, replacement of signs, etc., as deemed necessary by your maintenance personnel or the Fire Department. 8.
