The checklist shown below specifies the information which is required to be submitted as part of the site plan
application. The Department of Planning & Development will review the application to ensure that it is
complete prior to accepting it. If any portion of the application is not complete, it will be returned to the
applicant(s).
(1) One (1) set of approved comment sheets are required from each relevant review
agency prior to final approval of a site plan. It is recommended that applicants
contact the Department of Planning & Development to determine which review
agencies are relevant to their site plan application. A list of potentially relevant
review agencies is shown below:

Per Evan Wyatt's and my conversation with you yesterday concerning the two (2) office buildings on the
Advanced Mortgage Services site, our client wishes to add additional ingress/egress stairs for safety (as
shown on the enclosed exhibit) and turn the offices into condo offices. '[here would be no change in the
office mix or parking.
Advanced Mortgage Services is looking to complete an as -built change only for the staircases added to the
office buildings.

costs we incur will be billed to you monthly.
Please ensure a copy of the approved permit assembly is kept at the job site whenever working
within the Route 1129 right-of-way. I would also like to remind you of the need to erect and
maintain proper traffic control in accordance with our Virginia Work Area Protection Manual.
You are required to contact Mr. Dwight Hawkins at (540) 984-5616 at least 48 hours in
advance of the start of construction to arrange for any necessary inspections.

Buildings shall comply with The Virginia Uniform Statewide Building Code and section, 304, use
group B (Business), of the International Building Code/2000. Other Code that applies is CABO
A 117.1-98 Accessible and Usable Buildings and Facilities.
HC parking spaces and unloading shall not slope greater than 2% across area. Please confirm
slopes are in compliance on the accessible route from the parking to the structure. All required
egress doors shall be accessible to a min. exterior area for assisted rescue, section
1003.2.13.7.IBC. They are not shown on the plan at this time.
This type structure shall be subject to special inspections according to chapter 17 of the IBC.,
(soils, concrete, etc.).

We have received your revised site plan dated April, 2004 for the referenced project. It appears all of our
earlier review comments have been satisfactorily addressed. Please advise the owner of our approval. Also
please provide two (2) additional sets of approved construction plans with signed seal for VDOT
distribution.
The appropriate land use permit may now be applied for to cover work within the Route 1129 right-of-way.
The permit is issued by this office and will require a minimum processing fee, surety bond coverage, and the
salary & expenses of a State assigned Inspector. A copy of any/all recorded plats of dedication for drainage
easements or additional right-of-way required for implementation of this proposed project should be provided
to VDOT prior to issuance of any land use permits. Dedication should be made to the Commonwealth of
Virginia.
Once satisfactory application has been made, a permit will normally take approximately thirty days to process
and issue. Should you have any questions, do not hesitate to call.

The Potomac Edison Company, dba Allegheny Power, has reviewed the
construction plans for the above referenced project. The development will encroach upon
our Mead owbrook-Stonewall/Meadowbrook-West Winchester 138KV-transmission lines
RighT-of-Way. The Right -of -Way was acquired from James M. Horton, et al and of record
in Dr.--ed Book 583, Page 585, in the Frederick County, Virginia Land Records, as shown on
yr,,,r pi;in;. The following conditions should be noted in regard to our Right -of -Way:
1. No buildings or other structures are not allowed with our right-of-ways.
2. No equipment shall be operated within fifteen (15') feet of any energized overhead
conductor of the Company.
3. Any grading within our Right -of -Way is only allowed with our prior written approval.
The grading as shown on your plans referenced above is approved as shown. If any
further changes, would you please have a copy submitted to me, for our review and
approval.
4. We note that your landscaping plans show the proposed planting of Eastern Red
Cedars both within and close to our r/w, These trees have a mature growing height
of greater than fifty- (50) feet. We strongly suggest that trees planted within or
near the r/w have a maximum mature height of no more than twenty- (20) feet.
Our r/w agreement allows us to cut and remove trees both within and outside our
r/w. We would eventually remove and/or cut trees that have a mature height of
-greater than twenty- (20) feet.
