GENERAL CONSTRUCTION NOTES 1. The contractor shall furnish all materials, labor and equipment and perform all work including restoration for the completed installation of the utility systems and all other Improvements shown or implied as necessary for the completed facility ready for use. Unless otherwise noted specifications for all work to be in accordance with applicable Standards and Construction Specifications of The Virginia Department of Transportation, Virginia Department of Health, Frederick County Sanitation Autho�'{y and the contract documents. 2. The engineer has attempted to locate existing subsurface utilities; however, such may exist that are not shown. The contractor shall exercise care in this work so as to avoid damage to any utilities. Any damage shall be thp contractor s responsibiiifry The contractor shall notify all utility companies 72 hours prior to construction. Call Miss Utility i-W-552-7 1. 3. The contractor shall be responsible for all constriction stakeout. All dimensions, elevations and locations shall be verified in the field by each contractor prior to beginning the work. The Engineer will approve all cut sheets. 4. Any unusual subsurface conditions encountered during the course of the work shall be immediately brought to the attention of the Engineer and/or the Owner. 5. Contractor to obtain, at his expense, all permits required of this work and shall faithfully adhere to the permit conditions imposed. 6. All grading for improvements to be confined to the project area. Any work performed offsite is the Contractors responsibility. 7. Cutsheets shall be submitted to the Engineer for approval prior to ordering structures and start of construction. All cutsheets should show existing ground elevations at centerline of utility, structure or road, and offset hub elevation. FREDREICK COUNTY SANITATION AUTHORITY 1. The Authority's Water and Sewer Standards and Specifications are available at .fcso-water.com . 2. The water and sanitary sewer portion of these plans are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrange for inspection by, the Authority. 5. When on existing water main is tapped for the installation of a new line: a The Authority shall furnish all labor and material. b The developer/contractor must: 1. make application and pay deposit 2. mark the tap location 6. The contractor shall tie-in a new sewer line to an existing manhole by core drilling the manhole 7. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's Inspector. 8. The Authority's maintenance department shall furnish and install all water meters through 2" in size. It is the developer s/contractors responsibility to have the meter box assembly installed correctly. Before a permanent meter is installed: a The meter box, with its frame and cover, must be properfyy aligned with the coppersetter. b The frame and cover shall be set to final grade. c The distance between the top of the cover and the coppersetter shall be between 18 and 21 inches. d All components of the meter box assembly shall be in proper working order. 9. For water and sewer services that connect to existing lines: a The location of the service lateral is to be coordinated with the AuthoV. s Engineering Assistant. b An Application for Service must be made and fees paid before work will begin. c The Authority shall furnish and install all 5/8", 3/4 and 1" water services. d For all 1 %" and 2" services, the Authority shall make the top and bring the piping to the property line. The contractor shall fumish and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals. 9. Any water service line must have a backflow prevention assembly (double check valve or RPZ, as required), The assembly must meet ASSE standard No. 1015 or 1013. 10. All fire lines shall have a fire service meter assembly and a backflow prevention assembly (detector double check valve or RPZ, as required). The assembly shall meet ASSE standard No. 1048 or 1047. Radio read remotes are required. FIRE & RESCUE 1. Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited. 2. Access for emergency vehicles must be maintained at all times during construction. Temporary street signs and addresses must be posted as construction begins. Parked vehicles and staging of construction materials are prohibited within 20' of fire hydrants. Fire Hydrants are not to be obstructed at any time. VIRGINIA DEPARTMENT OF TRANSPORTATION 1. All work on this project shall conform to the latest editions of the Virginia De rtment of Transportation (VDOT) Road and Bridge Specifications, and Standards, The Virginia Erosion and Sediment Control Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards, specifications or plans, the most stringent shall govern. 2. All construction shall comply with the latest U.S. Department of Labor Occupational Safety ilk Health Administration and VOSH Rules do Regulations. 3. When working on VDOT right-of-way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition of VOOT's work area protection manual. Furermore, all traffic control flaggers must be certified in accordance with VDOT's January, 1994 (or latest edition of) Road & Bridge Specifications, Section 104.04-C. 4. The developer shall be responsible for relocatingg, at his expense, any and all utilities, including traffic signal poles, junction boxes, controllers, etc. owned by V.D.O.T. or private utility company. it is the sole responsibility of the developer to locate and identify utility facilities or items which may be in conflict with the proposed construction activity. V.D.O.T approval of these plans does not relieve the developer of this responsibility. 5. Design features relating to construction or to regulation, control and safety of traffic may be subject to change as deemed necessary by VDOT. Any additional expense incurred as a result of angfield revision will be responsibility of the developer, 6. Prior to initiation of work, Contractor shall be responsible for acquiring all necessary VDOT land use permits for any work on VDOT right-of-way. 7. If required by the local VDOT Residency Office, a preconstruction conference must be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT pprior to initiation of work. S. Contractor shall nosy the local VDOT Residency Office when work is to begin or cease for any undetermined length of time, VDOT will also require 48 hours notice for any inspection. 9. The Contractor will be responsible for maintaining adequate access to the project from the adjacent public road- way through construction and maintenance of a construction entrance in accordance with the Vir inia ion Sediment Control Handbook, Sec 3.02. Furthermore, access to other properties affected by this project shall be maintained through construction. The Developer shall have, within the limits of the project, an employee certified by the Department of Conservation and Recreation in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures for proper installation and deficiencies immediately after each rainfall, at least doily during prolonged rainfall, and weekly when no rainfall event occurs. 10. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. 11. All water and sewer lines within existing or proposed VDOT right-of-way are to have minimum 36" cover and, when possible, to be installed under roadway drainage facilities at conflict points. 12. My unusual subsurface conditions encountered during the course of construction shall be immediately brought to the attention of the engineer and VDOT. Work shall cease in that vicinity until an adequate design can be determined by the engineer and approved by V.D.O.T. 13. All fill areas, borrow material and undercut areas shall be inspected and approved by VDOT Inspector prior to placement of fill. Where CBR testing is required, a V.D.O.T representative shall be present to ensure the somple obtained is representative of the location. When soil samples are submitted to private labratories for testing, the samples shall be clearly identified and labeled as belonging to a project which will be accepted by V.D.O.T. and that testing shall be conducted according to applicable V.D.O.T. standards. 14. All roadway fill, base, subsurface material and backfill of utilit�yyy//storm sewer trenches shall be compacted in 6" lifts to 957of theoretical maximum density as determined by. AASHT0 T-99 Method A, within plus or minus 27. of optimum moisture for the full width of any dedicated street right-of-way. At the direction of VDOT Inspector density tests performed by a qualified independent agency shall be conducted as required in the VDOT Road and Bridge Specifications. A copy of all tests shall be submitted to VDOT prior to final VDOT approval. 15. VDOT Standard CD and UD underdrains shall be installed where indicated on these plans and further where determined necessary by VDOT Inspector. 16. The installation of any entrances and mailboxes within any dedicated street right-of-way shall meet VDOT minimum design standards and is the developer's responsibility. 17. Prior to acceptance by VDOT of any streets, any required street signsge and/or pavement markings must be installed by the developer or, at VDOT's discretion, by VDOT on an account receivable basis. Stop signs shall be thirty (30) inches tall with high intensity sheeting. The bottom of the sign shall be six (6) feet above the edge of pavement and installed a minimum of four (4) feet from the edge of pavement. Sign posts shall be of a type found on V.D.O.T.'s approved list. A copy of this list may be obtained from the District Material Section. Contact the local V.D.O.T. residency office with any questions regarding siggn material and placement. 18. The developer must provide to the local V.D.O.T. office a list of all material sources prior to construction. Copies of all invoices for materials within any dedicated street right of way must be provided to the local V.D.O.T. residency office prior to acceptance of the work. Units and total prices may be obscured. 19. Aggregate bases and sub -bases shall be placed on the sub-grode by means of a mechanical spreader. Density will be determined using the density control strip as specified in V.D.O.T.'s Road and Bridge Sped ications (Section 304) and VTM-10. A certified compaction technician shall perform these tests. A certified copy of the test results shall be provided to the Residency Office. In addition to checking stom depths, a V.D.O.T. representative shall be notified and given the opportunity to be present during the construction and testing of the density control strip. 20. Asphalt concrete pavement shall be placed in accordance with the latest edition of V.D.O.T.'s Road and Bridge Specifications (Section 315). Density will be determined using density control strip as specified in Section 315 and VTM-76. A certified compaction technician shall perform these tests. A certified copy of the test results shall to provided to the Residency Office. In addition to checking stom depths, a V.D.O.T. representative shall be notified and given the opportunity to be present during the construction and testing of the density controlMiears p 21. Temporary and permanent erosion control structures and methods to be in accordance with the Virginia Erosion Control Guidelines and local ordinances and to be utilized during the construction period. 5. All erosion control structures shown or as required during construction shall be continuously maintained throughout the peril in which areas are denuded. Areas shown to be surfaced are to have stone base installed at the earliest possible date after establishing grade. No area is to remain denuded for more than 7 calendar days when construction is not in progress. 6. Contractor to clean up and restore disturbed areas immediately upon completion of pipe -laying in the affected area. 7. All unpaved areas of the site which are disturbed by this construction shall be provided with 6' topsoil, fertilized, seeded, compacted and mulched. Seed bed to be watered and cared for as necessary to provide a good lawn quality gross cover. Areas not graded shall be d'isced, leveled and seeded to achieve klwn conditions. The Authority's Water and Sewer Standards and Specifications are available at .feso-water.com . 2. The water and sanitary sewer portion of these plans are valid for two years from date of FCSA approval. 3. Water and sewer lines are to be constructed in accordance with these plans and shall meet all Authority requirements. 4. The contractor shall coordinate with, and arrange for inspection by, the Authority. 5. When an existing water main is tapped! for the installation of a new line: a) The Authority shall fumish all labor and material. b) The developer/contractor must:. 1. make application and pay deposit 2. mark the tap location 6. The contractor shall tie-in a new sewer line to an existing manhole by core drilling the manhole 7. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's Inspector. 8. The Authority's maintenance deportment shall fumish and install all water meters through 2' in size. It is the developers/contractors responsibility to have the meter box assembly installed correctly. Before a permanent meter is installed: a The meter box, with its frame and cover, must be properly aligned with the coppersetter. b The frame and cover shall be set to final grade. c The distance between the top of the cover and the coppersetter shall be between 18 and 21 inches. d All components of the meter box assembly shall be in proper working order. 9. For water and sewer services that connect to existing lines: a The location of the service lateral is to be coordinated with the Authority.s Engineering Assistant. b An Application for Service must be made and fees paid before work will begin. c The Authority shall fumish and install all 5/8', 3/4 and 1' water services. d For all 1 %' and 2' services, the Authority shall make the top and bring the piping to the property line. The contractor shall fumish and install the meter vault assembly. The Authority will install the meter. e) The Authority will install all sewer laterals. 9. Any water service line must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet ASSE standard No. 1015 or 1013. 10. All fire lines shall have a fire service meter assembly and a backflow prevention assembly (detector double check valve or RPZ, as required). The assembly shall meet ASSE standard No. 1048 or 1047. Radio read remotes are required. FIRE & RESCUE 1. Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited. 2. Access for emergency vehicles must be maintained at all times during construction. Temporary street signs and addresses must be posted as construction begins. Parked vehicles and staging of construction materials are prohibited within 20' of fire hydrants. Fire Hydrants are not to be obstructed at any time. VIRGINIA DEPARTMENT OF TRANSPORTATION 1. All work on this project shall conform to the latest editions of the Virginia Irtment of Transportation (VDOT) Road and Bridge Specifications, and Standards, The Virginia Erosion and Sediment ntrol Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards, specifications or plans, the most stringent shall govern. 2. All construction shall comply with the latest U.S. Department of Labor Occupational Safety & Health Administration and VOSH Rules do Regulations. 3. When working on VDOi right-of-way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition of VIOT's work area protection manual. Furl ermore, all traffic control flaggers must be certified in accordance with VDOT's January, 1994 (or latest edition off) Road & Bridge Specifications, Section 104.04-C. 4. The developer shall be responsible for relocating, at his expense, any and all utilities, including traffic signal poles, junction boxes, controllers, etc. owned by V.D.O.T. or private utiI4. company. It isthe sole responsibility of the developer to locate and identify utility facilities or items which may be in conflict with the proposed construction activity. V.D.O.T approval of these plans does not relieve the developer of this responsibility. 5. Design features relating to construction or to regulation, control and safety of traffic may be subject to change as deemed necessary by VDOT. Any additional expense incurred as a result of anyfield revision will be responsibility of the developer, 6. Prior to initiation of work, Contractor shall be responsible for acquiring all necessary VDOT land use permits for any work on VDOT right-of-way. 7. If required by the local VDOT Residency Office, a preconstruction conference must be arranged and held by the engineer and/or developer with the attendance of the contractor, various County agencies, utility companies and VDOT grior to initiation of work. . Contractor shall nosy the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT will also require 48 hours notice for any inspection. 9. The Contractor will be responsible for maintaining adequate access to the project from the adjacent public road- way through construction and maintenance of a construction entrance in accordance with the Vir inia ion & Sediment Control Handbook, Sec 3.02. Furthermore, access to other properties affected by this project shall e maintained through construction. The Developer shall have, within the limits of the project, an employee certified by the Department of Conservation and Recreation in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures for proper installation and deficiencies immediately after each rainfall, at least daily during prolonged rainfall, and weekly when no rainfall event occurs. 10. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. 11. All water and sewer lines within existing or proposed VDOT right-of-way are to have minimum 36' cover and, when possible, to be installed under roadway drainage facilities at conflict points. 12. An unusual subsurface conditions encountered during the course of construction shall be immediately brought to the atEention of the engineer and VDOT. Work shall cease in that vicinity until on adequate design can be determined by the engineer and approved by V.D.O.T. 13. All f71 areas, borrow material and undercut areas shall be inspected and approved by VDOT Inspector prior to placement of fill. Where CBR testing is required, a V.D.O.T representative shall be present to ensure the sample obtained is representative of the location. When soil samples are submitted to private labratories for testing, the samples shall be clearly identified and labeled as belonging to a project which will be accepted by V.D.O.T. and that testing shall be conducted according to applicable V.D.O.T. standards. 14. All roadway fill, base, subsurface material and backfill of utility//storm sewer trenches shall be compacted in 6' lifts to 95% of theoretical maximum density as determined by, AASHT0 T-99 Method A, within plus or minus 27. of optimum moisture for the full width of any dedicated street right-of-way. At the direction of VDOT Inspector density tests performed by a qualified independent agency shall be conducted as required in the VDOT Road and Bridge Specifications. A copy of all tests shall be submitted to VDOT prior to final VDOT approval. 15. VDOT Standard CD and UD underdrains shall be installed where indicated on these plans and further where determined necessary by VDOT Inspector. 16. The installation of an entrances and mailboxes within any dedicated street right-of-way shall meet VDOT minimum design standards and is the developer's responsibility. 17. Prior to acceptance by VDOT of any streets, any required street signage and/or pavement markings must be installed by the developer or, at VDDOOT's discretion, by VDOT on an account receivable basis. Stop signs shall be thirty (30) inches tall with high intensity sheeting. The bottom of the sign shall be six (6) feet above the edge of pavement and installed a minimum of four (4) feet from the edge of pavement. Sign posts shall be of a type found on V.D.O.T.'s approved list. A copy of this list may be obtained from the District Material Section. Contact the local V.D.O.T. residency office with any questions regarding s'The material and placement. 12 The developer must provide to the local V.D.O.T. office a list of all material sources prior to construction. Copies of all invoices for materials within any dedicated street right of way must be provided to the local V.D.O.T. residency office prior to acceptance of the work. Unity and total prices may be obscured. 19. Aggregate bases and sub -bases shall be placed on the sub -grade by means of a mechanical spreader: Density will be determined using the density control strip as specified in V.D.O.T.'s Road and Bridge Specifications (Section 3041 and -10_ A certified compaction technician shall perform these tests. A certified copy of the test results shall be provided to the Residency Office. In addition to checking storn depths, a V.D.O.T. representative shall be notified and given the opportunity to be present during the construction and testing of the density control strip. 20. Asphalt concrete pavement shall be placed in accordance with the latest edition of V.D.O.T.'s Road and Bridge Smeifrcations (Section 315). Density will be determined using density control strip as specified in Section 315 and VTM-76. A certified compaction technician shall perform these tests. A certified copy of the test results shall be provided to the Residency Office. In addition to checking storn depths, a V.D.O.T. representative shall be notified and given the opportunity to be present during the construction and testing of the density control p. 21.
