V V On August 27, 1982, we met for the purpose of inspecting a proposed well site for approval for a public water supply well. In accordance with Section 3.23.03 of the Commonwealth of Virginia Water- works Regulations, this letter is to advise that the proposed well lot, as approximately located on the attached section of Stephens City, Virginia geo- logical survey map and more specifically located on the attached site develop- ment plan dated September 1, 1982 is tentatively approved for construction of a Class II well. This approval is vPlid for a period of twelve (12) months. If construc- tion of the well has not commenced by August 27, 1983, a reinspection of the well lot will be required. The well must be located in the center of the proposed well lot. The well lot must be graded to divert surface runoff from the well and to prevent ponding on the well lot, and an allweather access road, public or private, must be provided to the well lot. During the construction of the well, it is requested that this office be notified of the date and approximate time that grouting of the well be accomplished. Please transmit this notification as soon as the information is available to you so someone from this office can be present, if possible. Upon completion of the well, a yield and drawdown test of a minimum duration of 12 hours is required. Development chemical samples must be collected near the end of the yield test and submitted to either the State Consolidated Lab- oratories or an approved private laboratory for analysis. The proper chemical sampling containers, which will be provided by this office, will consist of a plastic gallon cubitainer, and a pint glass bottle. The collection of these samples will provide a base history of all inorganic chemical constitutents in this water. Of course the only sample required is that for nitrate; there- fore, we will"delay your project only if we have not determined the nitrate concentration. In order to determine whether chlorination of the water is necessary or will be acceptable (reference 8.03), a bacteriological series must be per- formed. This series can be performed during the twelve (12) hour drawdown test starting the second hour after pumping begins and spacing samples at a minimum of one hour. Bacteriological sample bottles, which will be provided by this office, will contain result forms that must be properly filled out in order for the state lab to analyze the sample correctly. These instructions will be given to you before the drawdown test. O�l� As this is to be a direct delivery noncommunity waterworks plans and spe- cifications need not be prepared by a licensed professional engineer. You do need to provide legible sketches as to intended construction. Included with the submittal of plans and specifications must be a copy of the Water Well Completion Report, a copy of the drawdown test, as specified above, a recorded plat of the well lot with an access road or a plat plan and recorded deed and a recorded dedication document for the well lot stating that the well lot will be used only for waterworks appurtenances as long as the lot is utilized as a part of a waterworks. Enclosed is an example of a. dedication document that may be used as a guideline. Upon receipt of all required information and documents, this office will atudy all material for suitability of: -review. As this is a direct delivery system this office may issue an approval. Construction of the pumping, treatment or storage facilities should not commence until receipt of the construction permit. Upon completion of these facilities, two bacteriological samples must be submitted from the furtherest discharge tap to determine if the system is suitable to put into service. ` If we can be of further assistance, please do not hesitate to call.
