REMARKS:
The keyway/core trench for Dam 91 was being excavated with a John Deere track hoe until
competent, unrippable shale was encountered. FEV observed and confirmed the depth of
excavation as suitable, while equipment removal and malfunction prevented completion of
the total keyway.

dimensional accuracy of the work being inspected. FOUNDATION ENGINEERING OF VIRGINIA does
not assume responsibility for the lines and grades of the contractor's work. All grades
noted in this inspection are from data as reported to FOUNDATION ENGINEERING OF VIRGINIA
by the contractor or his representative.
The above items have been inspected and found to be in compliance with the requirements
of the Virginia Uniform Statewide Building Code, the approved plans and local code
requirements.
