Entrance Permit Check List
1. Detailed sketch or plans.
2. Location map (prefer print from quadrangle map showing location).
3. Sketch should show flow arrows of proposed and existing drainage
pipes and sizes. Also flow arrows along the existing road.
4. Master plan of future development, if available.
5. Commercial entrance plan assemblies should include grading and
drainage contours with computations to support the drainage design.
6. Type of development - number of town houses, square foot of business,
etc. Number units of development. Days of operation, hours of operation.
7. Trip generation traffic volume and source. Traffic volume of existing
intersecting road. Anticipated left and right turn volumes.
8. Grades and profiles of entrance roads including vertical and horizontal
sight distance. Also sight distance at entrances.
9. Proposed typical sections of entrance road and turn lanes off of
existing state route.
10. Pavement design proposed construction.
