Enclosed you will find Commonwealth of Virginia, Department of Health, Waterworks
Operation Permit No. 2069329 with an effective date of August 19, 1991. This
permit is the Leary Educational Foundation, Inc.'s authorization from the State
Health Commissioner to operate the Timber Ridge School public waterworks, located
in Frederick County, in accordance with Part 2 of the Waterworks Regulations.
You will note that this waterworks has a design capacity of 300 students/staff.
This permitted design capacity is the existing system usage and shall not be
exceeded.
Please note that the permit has been issued with the following variances:
1. The Timber Ridge School waterworks must be capable of metering
total water production by August 19, 1992.
2. The Timber Ridge School waterworks must establish and have in
operation a cross -connection and backf low prevention control
program by August 19, 1992.
We look forward to your continued cooperation in the maintenance and operation

DESIGN BASIS: 300 students/staff
DESCRIPTION OF SYSTEM:
The source of supply for this system consists of two (2) drilled wells, a 1000-
gallon hydropneumatic tank and two (2) 500-gallon hydropneumatic tanks.
Well No. 1 is located approximately one hundred (100) feet east of the school
office building. No well construction information is available. The extended
well casing terminates approximately eighteen (18) inches above grade and is
equipped with a pitless adapter. Water is pumped from the well by means of a
three (3) hp submersible pump into the 1000-gallon hydropneumatic tank, thence
into the distribution system. The extended well casing is unprotected while the
exposed hydropneumatic tank piping is housed in an eight (8)-foot by four (4)-
foot by eight (8)-foot concrete vault equipped with a two (2)-foot square access
hatch and sump pump.
Well No. 2 is located approximately sixty (60) feet west of State Route 699 and
adjacent to the dining hall access road. No well construction information is
available. The extended well casing terminates approximately twelve (12) inches
above a three (3)-foot square concrete pad and is equipped with a pitless
adapter. Water is pumped from the well by means of a submersible pump into the
two (2) 500-gallon hydropneumatic tanks, thence into the distribution system.
The extended well casing is unprotected while the exposed hydropneumatic tank
piping is housed in a nine (9)-foot by twelve (12)-foot by seven (7)-foot
concrete block vault equipped with a four (4)-inch floor drain and two (2)-foot
square access hatch.
The design capacity for this system was based on the following data:

Storage Capacity: (1/3)(1000 + [2][5001) = 667 gallons @ 200
gallons/connection = 4 connections
Existing: 300 students/staff
This system is storage and unknown source capacity limited; therefore, the design

RE: Proposed Arts And Crafts Pavilion For Timber Ridge School
Dear Mr. Thomas:
This letter regards our telephone conversation of March 25, 1991.
During that conversation, we discussed the requirements for
obtaining a building permit to construct an arts and crafts
pavilion. You told me that the Leary Educational Foundation has
submitted plans to this department that show the most recent
development and that a Master Development Plan for the entire site
is kept at your office. You also said that the Master Development
Plan will be updated within the next three months and that the
location of some of the proposed buildings (including the Arts and
crafts pavilion) will change from your original plan. I offer the
following comments:
1) The Master Development Plan for this site should be submitted
to this department. This will enable the County to have an
updated version of the proposed growth for the Timber Ridge
School.
2) The Leary Educational Foundation needs to provide an updated
version of the site plan submitted in 1988 that provides the
location of this pavilion. This should be accompanied by
review agency comments from the Building Inspections
Department and the Fire Marshal, a complete application and
the appropriate site plan review fee ($300).
3) Future review for development on this site will be expedited
if the proposed uses on the Master Development Plan are
brought in under the same site plan. In other words, you may
submit a revised site plan that shows the construction of five
buildings at one time instead of just one (as in this case).
This would enable you to receive building permits for these
buildings without having to submit five separate revised site
plans.
4) Any site plan submitted to our department should show the
location of the existing and proposed buildings, roadways,
parking areas, pedestrian access, light poles, well and septic

locations, utility locations and topography. This assist all
review agencies with their review.
5) As I stated in our conversation of March 25, 1991, I will work
with you to assist you in receiving your building permit for
this project. I will need verification from the Fire Marshal
that this structure meets all NFPA Codes, and that it is
accessible to any fire and rescue equipment.
I appreciate your willingness to work with Frederick County to
insure that the Timber Ridge School meets all applicable County
Codes. I offer my assistance with your future projects.

This proposal is,approved subject to the following.requirements,:
1. That this approval shall become' -void if construction is not completed within one
year from the date of approval.
2. That the owner is responsible for notifying downstream property owners before
construction of the proposed facilities.
3. That'the sewage'tieatment facilities shall be constructed in accordance with
"Regulations of the Board of Health" governing the disposal of sewage.
4. That the plans and specifications for these proposed facilities shall be prepared
by a registered engineer and submitted to the Local Health Department for review.
An approved copy of the plans and specifications shall be provided the Bureau

5. Chlorination equipment shall be an approved type and properly housed in a heated
and ventilated building. The chlorination facilities shall provide at least
30 minute retention time based on average flow, and have some satisfactory means
of solids disposition.
6. That the Local Health Department provide construction and maintenance supervision
over the proposed facilities, and that monthly repozfs of daily chlorine residuals
are submitted to the Local Health Department.
That State Water Control Board has required that post -aeration be provided for the
final effluent to effect 90% DO saturation.

In accordance with Title 62.1, Chapter 3.1, Article 4, Section 62.1-44.19,
Paragraph 2, of the State Water Control Law, we are advising you of the
standards of quality and treatment requirements necessary to prevent con-
travening such standards of water quality.
The nronosed discharge is listed in Section 11 of the standards for the
Potomac River Basin and is classified IVA.

pH Range: 6.5 - 9.5
Maximum,Temperature rise above natural: 5°F
Maximum Temperature: 87°F
Coliform Organisms: Fecal coliforms not to exceed a log mean of 1000/100 ml.
Not to equal or exceed 2000/100 ml. in more than 10% of
samples.
Stream Uses --Subclass
Waters generally satisfactory for use as public or municipal water supply,
secondary contact recreation, propagation of fish and other aquatic life
and other beneficial uses.
The staff believes that the treatment requirements necessary to prevent con-
travening water quality standards should produce an effluent not exceeding
35 mg/l in BOD5 and suspended solids. Post -aeration of the effluent to provide
for 90% D.O. saturation is necessary.
