This letter is to confirm the Frederick County Planning Commission's action at
their meeting of February 17, 1988:
Approval of Site Plan #031-87 of Middletown Elementary School, zoned A-2, 15
acres, for an elementary school, playground, and parking areas. This property is
located north of Middletown, with direct access off Route 11, to the west of
Route 11, between Route 11 and I-81, in the Opequon Magisterial District.

LOCATION: North of Middletown, with direct access off Route 11, to the west of
Route 11, between Route 11 and I-81
MAGISTERIAL DISTRICT: Opequon
ADJACENT LAND USE AND ZONING: Agricultural, residential, and institutional (Lord
Fairfax Community College) land use and A-2 (Agricultural General) zoning.
PROPOSED USE AND IMPROVEMENTS: An elementary school, playground, and parking
areas.
REVIEW EVALUATIONS:
Highway Department - See attached letter.
Health Department - No objection; central water and sewer required.
Sanitation Authority - 1) P.C4 master - meter material and installation per
city specs. 2) P.C2 - If water line in intersection of entry drive is to be
extended in future might think of tee, gate valve, and plug.
Middletown Fire Company - Request fire hydrant at school drive entrance and
Route 11 - it would be beneficial for water supply North and South along
Route 11. Concern about play areas in fire lane - can play areas be crossed?
Concern about type of construction of fire lanes - request minimum 6 inches
of stone on all fire lanes not to be paved. NOTE: Fire hydrant located in
front of school is not shown on water main Profile "B" at approximate station
4+00.

Inspections - Shall comply to Use Group E (Educational) of the BOCA Basic

Middletown Elementary School
Planning and Zoning - No objection. The elementary school is a permitted use
in the A-2 (Agricultural General) zoning district. All planning, zoning,
site planning, and parking standards have been met. All VDOT construction
plan comments will be incorporated in the VDOT construction plans and site
plan, if required.

Dear Mr. Pappas:
This letter is to acknowledge the above referenced has been reviewed and we are
returning one set of plans with all the revisions marked in red. In review of
the submitted plans, we have the following comments:
1. Standard CG-6 curb and gutter to be provided along the right turn lane
adjacent to Route 11 and around the radii of the street intersection.
The northeast radius should be reduced to 50' with.12' offset from
Route 11 edge of pavement and 36' taper.
2. Concrete paved ditch to be used in lieu of stone ditch behind the curb
and gutter along Route 11. This will probably require removing a
section of the existing stone wall and providing drainage easement for
the backslope.
3: Paved ditch and additional drainage easement will be required to carry
the entrance road drainage. around the radius to the proposed double
line of pipe under the connection:
4. The double line of 35"x24" pipe arch will need to be extended and the
ends mitered in lieu of end sections due to restrictive width of the
drainage ditch.
On the entrance road:
1. The normal roadway ditch to be increased to 16" depth.
2. The vertical curve at station 5+00 to be increased to obtain a minimum
240' vertical sight distance.
3. The pipes at.station 8+50 and left of station.12+25 to have flared end
sections in lieu of end walls.
4. Erosion control such as soil stabilization mat standard EC-3 to be
provided left and right between approximate stations 1+00 and 4+75.
5. Roadside ditches shall be incorporated for both cut and fill sections
to .channel runoff within existing right-of-way or permanent drainage
easement.

6. Pavement design will be determined when the Department of Transportation
is provided with projected traffic generated counts.

Dear Dr. Walker:
As a followup to our telephone conversation of August 10, 1987,
please be advised that the Town of Middletown has agreed to the
following conditions regarding availability of sewer service to the
new Middletown Elementary School located on Route 11, North of
Middletown.
(1) All expenses regarding installation and maintenance of
sewer lines, pump stations and other equipment will be
the responsibility of the Frederick County School Board.
(2) A sewer hookup fee of $2,000.00 will be paid prior to
service.
(3) A daily capacity of approximately 6,000 gallons per day
(GPD) has been approved by Town Council.
(4) Sewer billing will be based on the measured.water usage.
The fee is as follows:
First 3,000 gallons - $ 23.50
Each additional.1,000 - 2.65
There will be a 507. surcharge for out of Town service.
If there are any other questions regarding sewer service to the
school, please contact me.

We have completed our review of the submitted plans for the entrance and
service road to the proposed Middletown Elementary S:_:=l. We are returning
cne (1) set of the approved plans with the revised date of April 1, 1988.
Fy discussed, the final pavement design will be determined by CBR Tests
conducted by the Virginia Department of Transportation at the developer's
e=pense and will be required when the rough grading has been completed.
A standard Virginia Department of Transportation stop sign will be required at
thr i_.ters-cticn with Route 11, as well as a permit for the proposed entrance.

We have enclosed a copy of the minimum requirements and data for subdivision

Dear Mr. Pappas:
This letter is to acknowledge the above referenced has been reviewed and we are
returning one set of plans with all the revisions marked in red. In review of
the submitted plans, we have the following comments:
1. Standard CG-6 curb and gutter to be provided along the right turn lane
adjacent to Route 11 and around the radii of the street intersection.
The northeast radius should be reduced to 50' with 12' offset from
Route 11 edge of pavement and 36' taper.
2. Concrete paved ditch to be used in lieu of stone ditch behind the curb
and gutter along Route 11. This will probably require removing a
section of the existing stone wall and providing drainage easement for
the backslope.
3. Paved ditch and additional drainage easement will be required to carry
the entrance road drainage around the radius to the proposed double
line of pipe under the connection.
4. The double line of 35"x24" pipe arch will need to be extended and the
ends mitered in lieu of end sections due to restrictive width of the
drainage ditch.
On the entrance road:
1. The normal roadway ditch to be increased to 16" depth.
2. The vertical curve at station 5+00 to be increased to obtain a minimum
240' vertical sight distance.
3. The pipes at station 8+50 and left of station 12+25 to have flared end
sections in lieu of end walls.
4. Erosion control such as soil stabilization mat standard EC-3 to be
provided left and right between approximate stations 1+00 and 4+75.
5. Roadside ditches shall be incorporated for both cut and fill sections
to channel runoff within existing right-of-way or permanent drainage
easement.

6. Pavement design will be determined when the Department of Transportation
is provided with projected traffic generated counts.

Dear Dr. Walker:
As a followup to our telephone conversation of August 10, 1987,
please be advised that the Town of Middletown has agreed to the
following conditions regarding availability of sewer service to the
new Middletown Elementary School located on Route 11, North of
Middletown.
(1) All expenses regarding installation and maintenance of
sewer lines, pump stations and other equipment will be
the responsibility of the Frederick County School Board.
(2) A sewer hookup fee of $2,000.00 will be paid prior to
service.
(3) A daily capacity of approximately 6,000 gallons per day
(GPD) has been approved by Town Council.
(4) Sewer billing will be based on the measured water usage.
The fee is as follows:
First 3,000 gallons - $ 23.50
Each additional 1,000 - 2.65
There will be a 50% surcharge for out of Town service.
If there are any other questions regarding sewer service to the
school, please contact me.
