Box 1027 Stephens City, VA 22655 Dear Sonny: Ref: Temporary Construction Entrance A's Copier Route 1200 Approx. 250' S. of Rte. 1322 Frederick County This is to acknowledge receipt of your letter dated July 26, 1990 regarding your request to construct a temporary construction entrance at the above referenced location. As requested you are hereby given permission to construct the temporary entrance. This letter is to be considered your permit to do so. The following are to be a part of this permit agreement: 1. while working on the State's right-of-way it will be necessary to use "Men Working" signs which must be erected prior to starting work and removed daily. 2. Traffic control will be in strict accordance with the attached. 3. Positive drainage is to be maintained within the right-of-way at all times. 4. All work on the State's right-of-way is to be confined to the temporary entrance location only. 5. A minimum of 8" of coarse aggregate by 40' in width will be required to be placed from the edge of pavement and extended back onto private property far enough to prevent carrying of mud onto the roadway. 6. Any damages to the pavement structure of Route 1200 adjacent to this entrance will be corrected by your firm to the satisfaction of the Virginia Department of Transportation. 7. The Department of Transportation will not be held liable for suit should such result from this operation. 9. This temporary construction entrance permit is being issued for 60 days. It should be understood a land use permit application will be submitted for the proposed commercial entrance to this office for review and approval within the sixty (60) day period. This permit will require an inspection fee and surety bond coverage in the amount of $5,000.00. If these requirements are not followed then this temporary construction entrance permit will be revoked. For your information this Department has not approved the site plan for this project. The following issues need to be addressed prior to final site plan approval: 1) The parking space dimensions need to be altered to provide for the required 180 square foot minimum. 2) VDOT approval is still required. 3) Engineer comments from Donohue need to be addressed. Once these issues have been addressed, I can recommend approval of this site plan. Please resubmit four (4) copies of the revised plan for review and approval. Box 601 Winchester, VA 22601 - ENGINEERS Re: A's Copier Soil Erosion Plan ARCHITECTS 17555.100 SCIENTISTS Dear Mr. Miller, We have reviewed the above referenced plan and offer the following comments: EROSION AND SEDIMENTATION CONTROL 1. A temporary construction entrance to the site is required. 2. A complete erosion and sedimentation control narrative describing construction phasing, soil stability, maintenance, final removal of measures and final stabilization will have to be included in the notes. 3. I do not agree with the pre -developed C of 0.35 and therefore do not feel that the runoff increase will only bee 1.54 C.FS for the post development. Also, in my opinion the pre -developed Tc will be higher than 5 minutes. This site drains to the Abraan's watershed and I feel that stormwater management is advisable. 4. It appears that some additional protection will be required at the end of the ditch line where the check dam is proposed to ensure that erosion does not by-pass check dam. A diversion dike instead of silt fencing is advised. 5. Straw bales are not recommended in the conveyance channel to the east of the site. I recommend check dams with silt traps in these areas. 6. Although this is not a comment pertaining to the erosion protection. I would recommend the Engineer enhance the drainage swale around the western building corner.
