This site plan calls for the construction of an addition of 3,000 square feet to the existing office facility, the extension of a concrete sidewalk, the provision of three disabled parking spaces with appropriate signage, the installation of a new sewage disposal system, and all appropriate erosion and sedimentation control measures. Enclosed are two copies of the approved site plan. This will enable you to provide a copy to the contractor and maintain a copy for your records. Our department received a letter, dated May 3, 1994, from the Lord Fairfax Environmental Health District (LFEHD). The purpose of this letter was to inform Frederick County that a new sewage disposal system will be required for this site, and that the LFEHD has no objection to the commencement of building construction. It is my understanding that Lewis & Associates is working with Shenandoah Gas, Greenway, Inc., the LFEHD, the Department of Environmental Quality, and the Office of Water Programs to resolve all requirements associated with the new sewage disposal system. Frederick County will sign -off on all building permits for this building addition. However, the new sewage disposal system must be installed and approved by the LFEHD prior to the issuance of a final Certificate of Occupancy Permit for this project. Hopefully this will assist Shenandoah Gas in the completion of this project. Our department will provide you with approved site plans once we have received the new sheet depicting the work associated with the new sewage disposal system. The following comments need to be adequately addressed prior to final site plan approval: 1) Please provide a signature on the Land Surveyor Certificate. 2) Please revise the FAR calculations as indicated on the marked -up plan. 3) Please revise the parking calculations as indicated on the marked -up plan. 4) Please provide the dimension of the existing parking spaces. 5) Your plan calls for three disabled parking spaces. Please provide the location for the third disabled parking space. All disabled parking spaces should have appropriate signage that complies with the ADA requirements. 6) Please provide the side setback as indicated on the marked -up plan. 7) The trash receptacle should be enclosed by a solid fence with a latched gate to prevent trash from exiting the contained area. 8) Please provide the finished floor elevation for the proposed office addition, as well as spot elevations for all corners of the proposed addition. 9) Is the pedestrian walkway at the same elevation as the paved parking area? If it isn't, provisions should be made to create a ramp for access from the customer parking lot. 10) Please ensure that the CNG Station is adequate for public use. It appears that there may be a conflict between employee parking and customer use. Please revise your site plan to address these comments. I will need to receive the site plan application, the site plan review fee, all approved review agency comments, and five copies of the final site plan in order to proceed with approval of this project. A sewage disposal repair permit will be issued as soon as plans and specifications are submitted and approved and approval is received from the Department of Environmental Quality and the Office of Water Programs. Neither of these offices should have any objection to the project, since the permit is to be issued to repair the existing septic system which was failing. The Certificate of Occupancy for the new construction should not be issued until the repair sewage disposal permit is issued, installed, and approved. Shenandoah Gas is presently required to continue to have an approved pumper pump and haul their septic tank effluent to eliminate any surface discharge.
