Dear Mr. Sweeney:
The Frederick County Planning Department administratively approved Site Plan #017-94 for the
Stephens City Urgent Care Facility on August 5, 1994. The approved site plan calls for the
construction of a medical office that will be 5,610 square feet, the installation of a paved parking
lot complete with curb and gutter, 38 regular parking spaces, 2 disabled parking spaces with
appropriate signage, perimeter parking lot landscaping, and marked fire lanes, the installation
of a five foot wide concrete pedestrian walkway with disabled ramps, the construction of a
concrete dumpster pad that is enclosed with a six foot high opaque wooden fence with a latched
gate, the provision of a freestanding business sign that is at least ten feet from the property line
and is a maximum of fifty square feet in size, the installation of a four foot high wooden fence
along the eastern parking lot to mitigate vehicle traffic lights from shining on residential
properties, the creation of a 24 foot wide access connection with the adjoining McDonald's
parking lot, and all appropriate erosion and sedimentation control measures.
I have provided Mr. Steve Gyurisin, of Gilbert W. Clifford & Associates with nine copies of
the approved site plan for distribution. Please ensure that an approved copy is provided to the
contractor and is maintained on -site. Please contact this office to schedule an on -site inspection
once all improvements are complete.

1) Please provide a signed Registered Professional Engineer Certificate on sheet 1 of 3, as

During our meeting, the Fire Marshal and the Stephens City Fire Chief expressed a concern
regarding a situation that is created by this use. In particular, a policy exists which requires the
Frederick County Emergency Services to transport patients from the Urgent Care facility on a
regular basis. This policy is creating a negative impact upon fire and rescue services. It would
be prudent for Gilbert W. Clifford & Associates to organize a meeting between representatives

of the Winchester Regional Health Systems and representatives of Emergency Services.
Hopefully, this will allow both parties to come to some reasonable conclusion regarding this
policy issue.
It should be noted that this is a policy issue and not a design issue. Frederick County will place
this site plan on the Planning Commission's bimonthly report for information. Our department
will provide administrative approval for this plan if the Planning Commission does not ask for
formal review and if all design standards are met. We will not put the Planning Commission
in the position of resolving a policy issue, as the property is legally zoned for the proposed use.
I have provided you with a copy of my marked plan for your use. Please provide our department
with the site plan application, the site plan review fee, and all review agency comments. This
will allow our clerical staff to create a file for this project and place it on the Planning
Commission's bimonthly report. Please provide our department with five copies of the final site
plan that adequately addresses all review agency comments for final approval.

As discussed recently, I am enclosing information you will need to make application for a permit to
cover improvements on the State's right-of-way in conjunction with the referenced project.
1. Land Use Permit Application #40190 (Form CE-7A)
► Land Use Permit Surety Bond (Form MP-20)
► Irrevocable Letter of Credit Bank Agreement (Form MP-231)
You will need to complete the application where indicated and return it along with six (6) sets of the
approved site plan, bond in the amount of S2,500.00, and fee in the amount of $45.00. Your check
should be made payable to the Virginia Department of Transportation. A recorded copy of the
required drainage easement deed and plat will also be needed.
Once satisfactory application has been made, a permit of this nature normally takes approximately
thirty (30) days to process and issue. Please understand no work can be performed on the State's
right-of-way until after your permit has been processed and issued.

As requested, we have reviewed the referenced project's site plan. Our comments may be found
on the enclosed plans marked in red and as follows:
1. The proposed connection to the McDonald's parking lot should be eliminated. If this
connection were allowed, cut through traffic via the McDonald's entrance on Route 277
to bypass the signalized intersection of Route 277/647 may become a problem.
2. A standard ES-1 flared end section should be considered on the outlet end of Culvert "B"
in lieu of the proposed endwall.
3. A standard CG-12 handicap ramp should be provided at the intersection of Route 647 and
Hyde Court.
4. The general notes should be revised as noted.
Please revise and resubmit six (6) copies for final approval. Should any changes be deemed
necessary, please design them to meet or exceed the above recommendations.

We are in receipt of the above referenced project's revised site plan dated
June 27, 1994. It appears all of our earlier comments have been satisfactorily
addressed.
The plan is therefore approved contingent upon an additional drainage easement
dedicated to Frederick County being provided to enclose a portion of the proposed
stormsewer system. The exact location is marked in red on the enclosed plans.
The developer may now apply for a land use permit to cover the proposed work within
the Route 647 right-of-way. Said permit will require a minimal processing and
inspection fee and surety bond coverage. Once satisfactory application has been
made a permit of this nature normally takes approximately thirty (30) days to process
and issue. In order to process the permit application we will require six (6) additional
sets of the revised plans and also a copy of the recorded drainage easement deed and
plat.

Dear Tom:
We have completed our review of the revised site plan dated
June 30, 1994 and offer the following comments:
1. Sheet 2 of 3: The proposed site development plan indicates
a 14' X 12' dumpster pad. Consideration should be given to
installing rigid pavement in front of this pad to accommodate the
heavy wheel loads of the trash trucks.
2. Sheet 3 of 3: The revision included on the June 30, 1994
drawings have adequately addressed a majority of my comments
related to storm drainage. However, the q(10) for the existing 24"
culvert should be revised to reflect the increases due to the
proposed development. The actual Q(des) appears to be more than
adequate.
It will not be necessary to resubmit the proposed site plan.
However, we would appreciate a final revised set of plans for our
records.
