A VDOT review has been conducted for the Valley Health Surgery Center Site Plan with a signature
date of October 12, 2016. Based off of the review of the Drawings, all of the previous VDOT
comments have been addressed and the Site Plan is approved by our office. Please ensure that all
applicable agencies have the most recent set of approved Drawings.
We offer the following general comments:
Our review and comments are general in nature. Should details be overlooked during plan review or
conditions in the field exist such that additional measures are warranted, such measures shall be
completed to the satisfaction of the department.
• Materials used and methods of construction shall adhere to the current observed VDOT Road and
Bridge Specifications, Road and Bridge Standards, Manual on Uniform Traffic Control Devices, and
Land Use Permit Special Provisions.
A preconstruction conference shall be held by the engineer and/or the developer with the attendance
of the contractor, various County agencies, utility companies, and VDOT prior to initiation of work.
• All drainage is to be carried within the right-of-way in ditch lines or gutters along the street to a pipe or
drainage easement.
• Any construction related charges to the approved plan must come through the design engineer to
VDOT for approval. Please allow a minimum of 5 business days for VDOT review.
A Land Use Permit shall be obtained before any work is performed on the State's right-of-way. The
permit is issued by this office and may require application fees, the salary & expenses of a State
assigned inspector, and surety bond coverage.
• Please submit one electronic set of the plans to this office for file and permitting.

Building shall comply with The Virginia Uniform Statewide Building Code and Section 304 Business of
the International Building Code/2012. Other codes that apply are ICCI ANSI Al 17.1-2009 Accessible
and Usable Buildings and Facilities and Virginia Energy Code 2012.

Building full NFPA 13 system installed and FDC located on plans.
All required exits shall be accessible. Maximum threshold at doorways shall not exceed 1/2".
All public entrances shall be accessible and comply with the 2012 Virginia Energy Code.
Van accessible and accessible parking and unloading shall be provided as required per Table 1106.1.
Hospital outpatient facilities require 10% and Rehabilitation and outpatient facilities require 20%
accessible parking. Please verify the required percentage of accessible spaces in relation to the
proposed Use Group.
Van accessible parking spaces shall measure 8' in width with an 8' unloading area or 11 ' parking width
with a 5' unloading area. All accessible spaces shall be identified correctly.
Accessible parking signage for van shall include $500 fine. See USBC Section 1106.8 for van signage.
Medical facilities shall be provided with an accessible passenger loading zone. Loading zone shall
comply with ANSI' Al 17.1-09. A minimum of 60" deep x 20' in length shall be marked and identified as
the unloading area. Please provide the location of the stri;vinglunloading on the plains.
Please note the requirements in Chapter 17 of USBCABC for special inspection requirements on the type
of structure( Soils, concrete etc.) Compaction/soil bearing/soil testing shall be required for placement of
new facility. A geotechnical evaluation shall be performed and submitted with application as required
USBC/IBC Section 1803.
