The contractor shall famish all materiols, labor and equipment and perform all work including restoration for the completed installation of the utility systems and all other improvements shown or implied as necessary for the completed facility ready for use. Unless otherwise noted, sppccifications for all work to be in accordance with applicable Standards and Construction SpecMcetions of es Yr inia Departmerd of Transportation, Virginia Department of Health, Frederick County Sanitation Authord� and the contract documents. 2. The engmeer has o erupted to locate on subsurface utilities; however, such may exist that are not shown. The contractor shall exercise cere in this work so as to avoid damage to any utilities. The contractor shall be responsible for all censtruction stakeout. All dimensions, elevations and locations shall be verified in the field by each 0, actor prior to beginning the work The Engineer will approve all cut sheets. 4. Any unusual subsurface conditions encountered during the course of the work shall be immediately brought to the attention of the of and/or the Owner. 5. Contractor to obtain, at his expense, all permits required of this work and shall faithfully adhere to the permit conditions imposed. 6. All grading for improvements to be confined to the project area. Any work performed offsite is the Contractoes responsibility. 7. Cutsheets shall be submitted to the EnGineer for approval prior to ordering structures and start of construction. NI cutsheets should show existing ground elevations at centerline of utility, structure or road, and offset hub elevation. FREDREICK COUNTY SANITATION AUTHORITY 1. The Authority's Water and Sewer Standards and Specifications are available at www.fcsa-water.com. 2. The water and sanitary sewer portion of the plans are valid for three years from date of FCSA approval. 3. The contractor shall adhere to the standards and specifications in effect at the time of construction. 4. The contractor shall coordinate with and arrange for inspection by the Authority. 5. The contractor shall connect a new sewer line to an existing manhole by core drilling the manhole. 6. A new water (or forced sewer) main shall be connected to an existing main by a wet tap. The Authority shall furnish all needed material and make the top. The contractor shall make an Application for Wet Tap and pay the required fees. 7. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's inspector. & The Authority's maintenance division shall furnish and install all water meters through 2' in size. H is the contractor's responsibility to have the meter box assembly installed correctly. Before a permanent meter is installed. a. The meter box, with its frame and cover, must be properly aligned with the coppersetter. b. The frame and cover shall be set to final grade. c. The distance between the top of the cover and the coppersetter shall to be between 18 and 21 inches. it. All components of the meter box assembly shall be in proper working order. 9. For services that connect to existing lines. a. The Authority shall fumish and install., 1. all 5/8 x3/4",3/4 , 1 ',1 1/2' and 2" water services 2. all sewer services. b. The contractor shall. 1. coordinate the location of the service lateral with the Authoritys engineering assistant 2. make an Application for Service and pay the required fees. 10. All water service lines must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet ASSE standard No. 1015 or 1013. 11. All fire lines must have a backflow prevention assembly (detector double check valve or RPZ, as required). The assembly shall meet ASSE standard No. 1048 or 1047 Any exterior, privately owned fire line shall also have a fire service meter. This assembly shall be installed immediately before the backflow, prevention unit. Radio read remotes are required. 12 The Authority shall review the mechanical plan(s) for design and material approval of a buildings: a. domestic water meter and its backflow, prevention device, and/or its b. fire service lines backflow prevention device. 13. DEQ must also approve sewer pump stations. FCS4 requires a copy of DEQs Cerfjfrcate to Operate and a copy of the station's DEQ approved 0&M mori These documents must be received before substantial completion is issued and water meters released. FIRE & RESCUE 1. Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited. 2. Access for emergency vehicles must be maintained at all times duringg construction. Temporary street signs and addresses must be posted as construction begins. Parked vehicles and staging of construction materials are prohibited within 20' of fire hydrants. Fire Hydrants are not to be obstructed at any time. V4 The developer shall be responsible for relocating, at his expense, any and all utilities, including traffic signal poles, junction boxes, controllers, etc., owned by MOT or private / public utility companies, lt is the sole responsibility of the developer to locate and identify utility facilities or items that may be in conflict with the proposed construction activity. VDOT approval of these plans does not indemnify the developer from this responsibility. V5. Design features relating to field construction, regulation, and control or safety of traffic may be subject to change as deemed necessary by MT. Any additional expense incurred as a result of any field revision shag be the responsibility of the developer. V6 Prior to initiation of work, the contractor shall be responsible for acquiring all necessary VDOT knit use permits for any work within MOT right-of-way. The contractor shall notify the local MOT Residency Office when work is to begin or cease for any undetermined length of time. MT requires and shall receive 48 hours advance notice prior to any required or requested inspection. V9. The contractor shall be responsible for maintaining adequate access to the project from the adjacent public runaway via a construction entrance Mot is constructed and maintained in accordance with Section 3.02 of the Virginia Erosion and Sediment Control Handbook Furthermore, access to other properties affected by this project shall be maintained through carstruclon. The Deve'oper shall have, within the limits of the project, an employee calif ed by the Yrginia Department of Conservation and Recreation (MR) in basion and Sediment Control who shall inspect erosion and siflation control devices and measures on o continuous basis for proper installation and operation. Deficiencies shall be promptly rectified. V10. Contractor shall ensure adequate drainage is achieved and maintained an the site dump and at Me end of construction. Vt 1. All water and sewer lines within existing or proposed VDOT right-of-way shall have a minimum thirty-six (36) inches cover and when passible shaft be installed under roadway drainage facilities of conflict points. VIZ Any unusual subsurface conditions (e.g., unsuitable soils, springs, sinkholes, voids, avis, etc.) encountered dung the course of construction shall be immediately brought to the attention of the engineer and MOT. Work shall cease in that vicinity unlit an adequate design can be determined by the engineer and approved by VDOT. V13. AI 611 areas, borrow material and undercut areas shall be inspected and approved by a VDOT representative prior to placement and fill. Where CBR testing is required, a VDOT representative shall be present to insure the sample obtained is representative of the location. When soil samples are submitted to private laboratories fa testing, the samples shall be clary identified and labeled as belonging to a project to be accepted by VDOT and that testing shall be performed in accordance with all applicable VDOT standards and procedures. Minor REMAC , PARKING LO V14. All roadway fill, base, sublimate material, and backlill in utility/storm sewer trenches shall be compacted in six (6) inch lifts to 95% of theoretical maximum density as determined by AkWO T-99 Method A. within plus or minus 2% of optimum moisture for the full width of any dedicated street right-of-way. At the direction of MOT, density tests shall be performed by a qualified independent agency in accordance with VDOT Road and Bridge Specifications. Gertthed copies of test reports shall be submitted to VDOT daffy unless specified otherwise. V15. MOT Standard CD and UO undendrdms shall be installed where indicated on these plans and/or as specified by Mr.. VI6. The installation of any entrances and mailboxes within any dedicated street right-oT-way, shall meet MT minimum design standards and a Me responsibility of Me developer. V17. Prior to VDOT acceptance of any streets, all required street signage ala/or povement markings shall be installed by the developer or at MOT's discretion, by MT on an account receivable basis following the Manual On Uniform Traffic Control Devices. V18. The developer shall provide the MOT Residency Office with a list of of material Bounces prior to the start of construc6'm. Copies of all invoices for materials utffized within any dedicated street right-of-way must he provided to the local MT Residency Office prior to acceptance of the work. Unit and total prices may be obscured. V19. Aggregate base and subbase materials shall be placed an subgrade by means of a mechanical spreader. Density wig be determined using the density control slop in accordance with Section 304 of the VDOT Road and Bridge Specifications and VIM -10. A certified compaction technician shall perform these tests. Certified copies o/ test reports shall be submitted to VDOT daffy, unless spelled otherwise. In adddian to checking stone depths, a MOT representative shall be notified and given the opportunity to be present during the construction and testing of Me density control strip. V20. Asphalt concrete pavements shall be placed in accordance with Section 315 of the VDOT Road and Bridge Speclicatim.. Density she// be determined using the density control ship as specified in Section 315 and VIM -76. A certified compaction technician shall perform these tests. Cer6fed copes of test reports shall be submitted to MT daily, unless specified otherwise. A VDOT representative shag be notified and given the opportunity to be present during the construction and testing of the control stn'p. M. In accordance with Section 302.03, the foundafon for pipe culverts thirty-six (36) inches and lager shall be explored below the bottom of the excavation to determine the type and condition of the foundation. The contractor shall report findings of foundation exploration to the engineer and MOT fa approval prior to pacing pipe. Foundation designs shall campy with MOT Road and Bridge Standard PB -1. Where soft, yielding, or otherwise unsuitable foundation is encountered, the foundation design and/or need to foundation stabifrzabon sholl be determined by the engineer and approved by MT. V22. The foundations for all box culverts shag be investigated by means of eyplorotory borings advanced below proposed foundation elevation to determine the lips and condition of the foundation. The contractor shall submit copies of borehole hgs and report findings of foundation explorrdan to Ore engineer and MOT for approval prior to constructing box. Foundation designs shall comply with MOT Road and Bridge Standard P16-1. Contrary to Me Standard, where rock is encountered and cast -in-place box is proposed, the thickness of bedding shall be six (6) inches Where soft, yielding, or otherwise unsuitable foundation is encountered, the foundation design and/a need for foundation stabilization shall be determined by the engineer and approved by MT. V23. Approval of these plans shall expire three (3) years from the date of the approval letter. V24. VDOT Standard CG -12 Curb Ramps shall be installed where indicated on these pans and/or as specified by MOT. V25. VDOT Standard Guardrail shall be installed where warranted and/a as proposed on these plans in accordance with Mrs installation criteria. Prior to initiating any land disturbing activity, it will be necessary to schedule an on-site pre -construction meeting with public works staff to discuss the approved erosion and sediment control plan. No pre -construction meetings will be scheduled prior to issuance of the land disturbance permit for the subject project. 2. Contractor to contact the public works department to schedule the required erosion and sediment control measures inspections as indicated by the program administrator. If the erosion and sediment control measurers have been installed in accordance with the approved plan, the Department of Public Works will grant approval to initiate land disturbing activities. 3. A letter from the engineer -of -record shall be included with the as -built which states that the subdivision has been constructed in accordance with the approved plan. c) Individual subdivision lot grading plans shall be submitted by the builder at the time of the building permit application for each lot proposed for construction. 4. Temporary and permanent erosion control structures and methods to be in accordance with the Virginia Erosion Control Guidelines and local ordinances and to be utilized during the construction period. 5. All erosion control structures shown or as required during construction shall be confinuousy maintained throughout the period in which areas are denuded. Areas shown to be surfaced are to have stone base installed at the earliest possible date after establishing grade. Contractor to clean up and restore disturbed areas immediately upon completion of pipe -trying in the affected area. 7.
