THE CONTRACTOR SHALL FURNISH ALL MATERIALS, LABOR AND EQUIPMENT AND PERFORM ALL WORK INCLUDING RESTORATION FOR THE COMPLETED INSTALLATION OF THE UTILITY SYSTEMS AND ALL OTHER IMPROVEMENTS SHOWN OR IMPLIED AS NECESSARY FOR THE COMPLETED FACILITY READY FOR USE. UNLESS OTHERWISE NOTED, SPECIFICATIONS FOR ALL WORK TO BE IN ACCORDANCE WITH APPLICABLE STANDARDS AND CONSTRUCTION SPECIFICATIONS OF THE VIRGINIA DEPARTMENT OF TRANSPORTATION, VIRGINIA DEPARTMENT OF HEALTH, FREDERICK COUNTY, , AND THE CONTRACT DOCUMENTS. 2. THE ENGINEER HAS ATTEMPTED TO LOCATE EXISTING SUBSURFACE UTILITIES; HOWEVER, SUCH MAY EXIST THAT ARE NOT SHOWN. THE CONTRACTOR SHALL EXERCISE CARE IN THIS WORK SO AS TO AVOID DAMAGE TO ANY UTILITIES. ANY DAMAGE SHALL BE THE CONTRACTOR'S RESPONSIBILITY. THE CONTRACTOR SHALL NOTIFY ALL UTILITY COMPANIES 72 HOURS PRIOR TO CONSTRUCTION. CALL "MISS UTILITY" 1-800-257-7777. 3. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ALL CONSTRUCTION STAKEOUT. ALL DIMENSIONS, ELEVATIONS AND LOCATIONS SHALL BE VERIFIED IN THE FIELD BY EACH CONTRACTOR PRIOR TO BEGINNING THE WORK. THE ENGINEER WILL APPROVE ALL CUT SHEETS. 4. ANY UNUSUAL SUBSURFACE CONDITIONS ENCOUNTERED DURING THE COURSE OF THE WORK SHALL BE IMMEDIATELY BROUGHT TO THE ATTENTION OF THE ENGINEER AND/OR THE OWNER. 5. CONTRACTOR TO OBTAIN, AT HIS EXPENSE, ALL PERMITS REQUIRED OF THIS WORK AND SHALL FAITHFULLY ADHERE TO THE PERMIT CONDITIONS IMPOSED. 6. ALL GRADING FOR IMPROVEMENTS TO BE CONFINED TO THE PROJECT AREA. ANY WORK PERFORMED OFFSITE IS THE CONTRACTOR'S RESPONSIBILITY. 7. CUTSHEETS SHALL BE SUBMITTED TO THE ENGINEER FOR APPROVAL PRIOR TO CONSTRUCTION. ALL CUTSHEETS SHOULD SHOW EXISTING GROUND ELEVATIONS AT CENTERLINE OF UTILITY, STRUCTURE OR ROAD, AND OFFSET HUB ELEVATION. 8. ALL PERMITS RELATING TO WETLANDS AND/OR WATERS OF THE UNITED STATES (W.O.U.S.) REQUIRED BY STATE AND/OR FEDERAL LAW WILL BE OBTAINED PRIOR TO CONSTRUCTION. 9. ALL UTILITIES (EXCEPT EXISTING ELECTRICAL LINES) TO BE PLACED UNDERGROUND. 10. UTILITY POLE RELOCATION TO BE COORDINATED WITH ALLEGHENY POWER DURING CONSTRUCTION. 11. ALL MATERIALS USED FOR FILL SHALL BE FREE OF UNSUITABLE MATERIALS SUCH AS WOOD, ROOTS, ROCKS, OR BOULDERS OF UNACCEPTABLE SIXE, OR ANY OTHER NON-COMPACTING SOIL MATERIAL. 12. MORRIS RITCHIE HAS NOT PERFORMED A WATER SYSTEM ANALYSIS FOR THIS SITE. A WATER SYSTEM ANALYSIS SHALL BE PERFORMED BY OTHERS AT THE TIME OF THE BOOSTER PUMP STATION DESIGN TO ENSURE THAT ADEQUATE FIRE FLOW IS PROVIDED AT ALL ON SITE HYDRANTS AND THE ROUTINE AND PEAK FLOW DEMANDS AS LISTED ON SHEET 2 ARE MET. 13. EXISTING SANITARY SEWER, SANITARY SEWER LATERALS, WATERLINE AND STORM SEWER SHOWN ON THE PLANS TO BE REMOVED SHALL BE DONE SO IN ACCORDANCE WITH FREDERICK WATER. THE CONTRACTOR SHALL BE RESPONSIBLE FOR COORDINATION WITH THE FREDERICK WATER SITE INSPECTOR TO COORDINATE THE REMOVAL OF ALL EXISTING UTILITIES. FIRE MARSHAL GENERAL NOTES 1. AN INSPECTION BY THE FIRE MARSHALL'S OFFICE WILL DETERMINE WHERE CURBING MUST BE PAINTED YELLOW. 2. "NO PARKING - FIRE LANE" SIGNS WILL BE PLACED ON THE SITE BY THE FIRE MARSHALL'S DIRECTION. 3. THE CONTRACTOR MUST ALIGN THE FIRE HYDRANTS TO THE DIRECTION AS DETERMINED BY THE FIRE MARSHALL. 4. BURNING OF LAND CLEARING DEBRIS REQUIRES A PERMIT FROM THE FIRE MARSHALL. BURNING OF CONSTRUCTION DEBRIS IS PROHIBITED. 5. ACCESS FOR EMERGENCY VEHICLES MUST BE MAINTAINED AT ALL AT ALL TIMES DURING CONSTRUCTION. TEMPORARY STREET SIGNS MUST BE POSTED AS CONSTRUCTION BEGINS. CONTRACTOR MUST ENSURE THAT PARKED VEHICLES DO NOT IMPEDE ACCESS TO FIRE HYDRANTS. FIRE HYDRANTS ARE NOT TO BE OBSTRUCTED AT ANY TIME. 6. IN LIEU OF A FIRE MARSHALL, A COUNTY BUILDING INSPECTOR WILL HANDLE ALL DUTIES. DEPARTMENT OF INSPECTION GENERAL NOTES 1. THE BUILDING(S) MUST COMPLY WITH THE VIRGINIA UNIFORM STATEWIDE BUILDING CODE AND THE BOCA NATIONAL BUILDING CODE/2006. OTHER CODES THAT APPLY ARE CABO A117.1-92 ACCESSIBILITY AND USABLE BUILDING & FACILITIES. 2. THE REQUIREMENTS IN BOCA MANDATE EITHER A FIRE WALL (SECTION 707) OR A FIRE SUPPRESSION SYSTEM IF THE AREA EXCEEDS 12,000 SQ. FT. 3. ARCHITECTURAL PLANS, SUBMITTED FOR BUILDING PERMITS, MUST SHOW ALL INGRESS/EGRESS LOCATIONS TO THE EXTERIOR. 4. MAXIMUM SLOPE ACROSS HANDICAP PARKING STALLS SHALL NOT EXCEED 2%. ZONING GENERAL NOTES 1. EXTERIOR LIGHTING WILL BE PROVIDED ON ARCHITECTURAL PLANS. 2. ALL LIGHTS WILL BE SHIELDED OR DIRECTED TO PREVENT GLARE ON NEIGHBORING PROPERTIES AND RIGHT OF WAYS. 3. ALL UTILITIES THAT WILL SERVICE THIS SITE MUST BE PLACED UNDERGROUND. FREDERICK WATER - CONSTRUCTION NOTES 1. THE AUTHORITY'S WATER AND SEWER STANDARDS AND SPECIFICATIONS ARE AVAILABLE AT WWW.FREDERICKWATER.COM. 2. THE CONTRACTOR SHALL ADHERE TO FREDERICK WATER STANDARDS AND SPECIFICATIONS IN EFFECT AT THE TIME OF CONSTRUCTION. 3. THE CONTRACTOR SHALL COORDINATE WITH AND ARRANGE FOR INSPECTION BY FREDERICK WATER. 4. THE CONTRACTOR SHALL CONNECT A NEW SEWER LINE TO AN EXISTING MANHOLE BY CORE DRILLING THE MANHOLE. 5. A NEW WATER (OR FORCED SEWER) MAIN SHALL BE CONNECTED TO AN EXISTING MAIN BY A WET TAP. FREDERICK WATER SHALL FURNISH ALL NEEDED MATERIAL AND MAKE THE TAP. AN APPLICATION FOR WET TAP SHALL BE SUBMITTED AND THE INITIAL FEE PAID BEFORE THE TAP IS MADE. 6. EXACT LOCATIONS OF WATER AND SEWER SERVICES ON NEW LINES ARE TO BE COORDINATED WITH FREDERICK WATER'S INSPECTOR. 7. FREDERICK WATER'S MAINTENANCE DIVISION SHALL FURNISH AND INSTALL ALL WATER METERS THROUGH 2 INCH IN SIZE. IT IS THE CONTRACTOR'S RESPONSIBILITY TO HAVE THE METER BOX ASSEMBLY INSTALLED CORRECTLY. BEFORE A PERMANENT METER IS INSTALLED: A. THE METER BOX, WITH ITS FRAME AND COVER, MUST BE PROPERLY ALIGNED WITH THE COPPERSETTER. B. THE FRAME AND COVER SHALL BE SET TO FINAL GRADE. C. THE DISTANCE BETWEEN THE TOP OF THE COVER AND THE COPPERSETTER SHALL BE BETWEEN 18 AND 21 INCHES. D. ALL COMPONENTS OFF THE METER BOX ASSEMBLY SHALL BE IN PROPER WORKING ORDER. 8. FOR SERVICES THAT CONNECT TO EXISTING LINES: A. FREDERICK WATER SHALL FURNISH AND INSTALL: 1. ALL 5 8"X3 4", 3 4", 1", 1 1 2" AND 2" WATER SERVICES 2. ALL SEWER SERVICES. B. THE OWNER/DEVELOPER SHALL: 1. COORDINATE (OR HAVE THE CONTRACTOR COORDINATE) THE LOCATION OF THE SERVICE LATERAL WITH FREDERICK WATER'S ENGINEERING ASSISTANT. 2. SUBMIT AN APPLICATION FOR SERVICE AND PAY THE REQUIRED FEES. 9. ALL WATER SERVICE LINES MUST HAVE BACKFLOW PREVENTION ASSEMBLY (DOUBLE CHECK VALVE OR RPZ, AS REQUIRED). THE ASSEMBLY MUST MEET ASSE STANDARD NUMBER 1015 OR 1013. 10. ALL FIRE LINES MUST HAVE A BACKFLOW PREVENTION ASSEMBLY (DETECTOR DOUBLE CHECK VALVE OR RPZ, AS REQUIRED). THE ASSEMBLY SHALL MEET ASSE STANDARD NUMBER 1048 OR 1047. ANY EXTERIOR, PRIVATELY OWNED FIRE LINE SHALL ALSO HAVE A FIRE SERVICE METER. THE ASSEMBLY SHALL BE INSTALLED IMMEDIATELY BEFORE THE BACKFLOW PREVENTION UNITY. RADIO READ REMOTES ARE REQUIRED. 11. FREDERICK WATER SHALL REVIEW THE MECHANICAL PLAN(S) FOR DESIGN AND MATERIAL APPROVAL OF A BUILDING'S: A. DOMESTIC WATER METER AND ITS BACKFLOW PREVENTION DEVICE, AND/OR ITS B. FIRE SERVICE LINE'S BACKFLOW PREVENTION DEVICE. 12. DEQ MUST ALSO APPROVE SEWER PUMP STATIONS. FREDERICK WATER REQUIRES A COPY OF DEQ'S CERTIFICATE TO OPERATE AND A COPY OF THE STATION'S DEQ APPROVAL O&M MANUAL. THESE DOCUMENTS MUST BE RECEIVED BEFORE SUBSTANTIAL COMPLETION IS ISSUED AND WATER METERS RELEASED. ROCK EXCAVATION: 1. ROCK SHALL BE EXCAVATED, WITHIN THE BOUNDARY LINES AND GRADES SHOWN ON THE PLANS. 2. THE TERM "ROCK" AS USED HEREIN SHALL INCLUDE ALL MATERIAL REQUIRING BLASTING, BARRING, OR WEDGING FOR IT'S REMOVAL FROM ITS ORIGINAL BED. 3. FOR EXCAVATIONS IN WHICH PIPE WILL BE LAID, THE ROCK SHALL BE EXCAVATED TO A DEPTH OF 12 INCHES BELOW THE LOWER OUTSIDE SURFACE OF THE PIPE BARREL OR BELL AND FILLED IN TO THE PROPER GRADE WITH SELECTED FILL MATERIAL PROPERLY GRADED AND COMPACTED TO PROVIDE UNIFORM SUPPORT FOR THE BARREL OF THE PIPE. THE FURNISHING, PLACING, COMPACTING, AND SHAPING OF THE SELECTED FILL MATERIAL SHALL BE INCLUDED AS PART OF THE ROCK EXCAVATION AND NO EXTRA PAYMENT WILL BE MADE THEREFOR. VERTICAL LIMITS FOR ROCK EXCAVATION SHALL BE AS SPECIFIED HEREINBEFORE. 4. FOR STRUCTURES, THE ROCK SHALL BE EXCAVATED ONLY TO THE BOTTOM OF THE STRUCTURE. 5. FOR PRECAST STRUCTURES, A MINIMUM OF 12" BEDDING IS REQUIRED. 6. WHERE APPLICABLE, OTHER REQUIREMENTS OF SECTION 8 OF THE CONSTRUCTION STANDARDS AND SPECIFICATIONS OF FREDERICK COUNTY, "EARTH TRENCH EXCAVATION," SHALL BE FOLLOWED. 7. THESE NOTES SHALL BE SUPERSEDED BY SPECIFICATION SECTION 02300-EARTH MOVING BLASTING: 1. BLASTING SHALL BE PERFORMED BY QUALIFIED PERSONNEL HOLDING THE REQUIRED LICENSE ISSUED BY THE COMMONWEALTH OF VIRGINIA FIRE MARSHALLS OFFICE. 2. A PRE-BLAST SURVEY SHALL BE MADE AVAILABLE TO ALL PROPERTY OWNERS WHOSE STRUCTURES ARE WITHIN THREE-HUNDRED (300) FEET OF THE BLAST ZONE. NOTIFICATION OF THE PROPERTY OWNERS IS THE RESPONSIBILITY OF THE DEVELOPER. A MINIMUM OF 48-HOUR NOTICE SHALL BE GIVEN. 3. THE TOWN PLANNING OFFICE SHALL BE NOTIFIED 48 HOURS PRIOR TO COMMENCEMENT OF BLASTING. 4. BLASTING SHALL BE PERFORMED AS DIRECTED OF APPROVED AS TO NUMBER, LENGTH, PLACEMENT, AND DIRECTION AND LOADING OF HOLES. THE CHARGES USED SHALL NOT MAKE THE EXCAVATION UNDULY LARGE OR IRREGULAR, NOR INURE MASONRY OR EXISTING STRUCTURES AT THE SITE OR IN THE VICINITY THEREOF. EACH BLAST SHALL BE COVERED WITH HEAVY TIMER OR STEEL MATS. EQUIPMENT USED FOR DRILLING OR HOLES SHALL HAVE A POSITIVE MEANS OF DUST CONTROL, SUBJECT TO THE ENGINEER'S APPROVAL. 5. WHENEVER THE ENGINEER DETERMINES THE FURTHER BLASTING MAY INJURE OR DAMAGE ADJACENT ROCK, MASONRY, OR OTHER STRUCTURES, BLASTING SHALL BE DISCONTINUED. IN SUCH CASE, THE REMAINING ROCK SHALL BE EXCAVATED BY BORING, WEDGING, OR OTHER APPROVED METHOD. 6. BLASTING SHALL NOT BE PERMITTED WITHIN 25 FEET OF COMPLETE PIPES OR STRUCTURES. WHERE SEWERS, GAS, WATER, STEAM, OR OTHER UTILITY DUCTS OR LINES, BASIN CONNECTIONS, OR OTHER STRUCTURES HAVE BEEN EXPOSED DURING EXCAVATION, SUCH STRUCTURE SHALL BE ADEQUATELY PROTECTED FROM DAMAGE BEFORE PROCEEDING WITH THE BLASTING. ANY STRUCTURE, PIPELINE, OR CONDUIT DAMAGED BY BLASTING SHALL BE PROMPTLY REPAIRED AT NO COST TO THE TOWN. ADDITIONAL BUFFER DISTANCE MAY BE REQUIRED FOR BLASTING DUE TO SITE CONDITIONS AND TYPE OF PIPE IN GROUND. INSPECTION BY THE PUBLIC WORKS DIRECTOR IS REQUIRED TO DETERMINE BLASTING SETBACKS. FORTY-EIGHT (48) HOURS NOTICE IS REQUIRED FOR THE INSPECTION. 7. BLASTING SHALL NOT BE CARRIED ON WITHIN 300 FEET OF ANY RADIO TRANSMITTER OR RADIO FREQUENCY EMISSION EQUIPMENT SUCH AS HIGH-FREQUENCY WELDERS. BLASTING CAPS SHALL BE KEPT IN TIGHTLY CLOSED, ALL METAL CANS WHEN IN THE VICINITY OF SUCH EQUIPMENT. 8. EXPLOSIVES A. A SUFFICIENT QUANTITY OF EXPLOSIVES SHALL BE KEPT ON THE SITE TO AVOID DELAY TO THE WORK, BUT AT NO TIME SHALL THERE BE A QUANTITY IN EXCESS OF THAT WHICH WILL BE REQUIRED FOR USE WITHIN THE FOLLOWING 12 HOURS. SUCH EXPLOSIVES SHALL BE STORED, HANDLED, AND USED IN CONFORMITY WITH ALL STATE AND LOCAL LAWS, ORDINANCES, AND REGULATIONS GOVERNING THE STORAGE AND USE OF EXPLOSIVES. THE CONTRACTOR SHALL TAKE OUT PERMITS AND EXECUTE A BOND THEREFOR, AS REQUIRED BY THE ORDINANCES OF THE JURISDICTION IN WHICH THE WORK IS BEING DONE, RELATING TO PERMITS AND BONDS FOR BLASTING. B. THE MAGAZINE KEEPER SHALL KEEP AN ACCURATE DAILY RECORD AND ACCOUNT FOR EACH PIECE OF EXPLOSIVE, DETONATOR, AND EQUIPMENT FROM THE TIME OF DELIVERY AT THE MAGAZINE UNTIL USED OR REMOVED FROM THE SITE. IN THE EVENT OF LOSS OF MISPLACEMENT OF BLASTING MATERIALS, THE CONTRACTOR SHALL IMMEDIATELY NOTIFY THE ENGINEER AND LOCAL AUTHORITIES HAVING JURISDICTION IN SUCH MATTERS. SINKHOLES AND KARST FEATURES: 1. ALL SINKHOLES OR OTHER KARST FEATURES IDENTIFIED PRIOR TO CONSTRUCTION SHALL BE EITHER MITIGATED OR SEPARATED FROM CONSTRUCTION IN ACCORDANCE WITH FREDERICK COUNTY ZONING ORDINANCE. 2. SINKHOLES OR KARST FEATURES IDENTIFIED DURING CONSTRUCTION SHALL BE MITIGATED AS DESCRIBED IN THE VIRGINIA DEPARTMENT OF TRANSPORTATION'S LOCATION AND DESIGN DIVISION INSTRUCTIONAL AND INFORMATIONAL MEMORANDUM 228 (IIM-LD-228), OR OTHER APPLICABLE STANDARD AS RECOMMENDED BY A PG OR PE AND APPROVED BY THE TOWN'S ENGINEER AND THE TOWN'S ZONING ADMINISTRATOR. 3. STORMWATER DISCHARGE INTO A KARST FEATURE SHALL NOT BE INCREASED OVER ITS PREDEVELOPMENT RATE. 4. STORMWATER RUNOFF FROM PAVED AREAS OR STRUCTURES SHALL NOT DIRECTLY ENTER A MITIGATED SINKHOLE OR OTHER KARST FEATURES. STORMWATER FACILITIES SHALL BE DESIGNED TO ROUTE RUNOFF THROUGH APPROVED VEGETATIVE FILTERS OR OTHER FILTRATION MEASURES BEFORE IT ENTERS A MITIGATED SINKHOLE OR OTHER KARST FEATURES. 5. AFTER NECESSARY TESTING TO DETERMINE IF SINKHOLES ARE PRESENT ON THE LOCATION OF A PROPOSED STORMWATER BASIN, STORMWATER BASINS IN KARST AREAS SHALL BE LINED WITH EITHER IMPERMEABLE SOIL OR A SYNTHETIC MEMBRANE TO PREVENT SUDDEN LOSS OF THE CONTENTS OF THE BASIN INTO THE GROUNDWATER DUE TO INDUCED COLLAPSE. WHERE NATIVE MATERIALS ARE DEEMED ADEQUATE FOR THE PURPOSE, THE GEOTECHNICAL ENGINEER SHALL VERIFY CONDITIONS, ESTABLISH THE REQUIRED PARAMETERS, AND MONITOR PLACEMENT. 6. CONSTRUCTION PLANS SHALL CONTAIN A NARRATIVE DESCRIBING STORMWATER DRAINAGE DESIGN, RETENTION, EROSION CONTROL, AND WHERE APPROPRIATE, STORMWATER QUALITY MITIGATION MEASURES, AS THESE PROVISIONS RELATE TO KARST FEATURES. 7. UNDERGROUND UTILITIES LOCATED WITHIN 100 FEET OF SINKHOLES AND KARST FEATURES SHALL BE LAID OUT SO THAT THEY' DO NOT INTERSECT THOSE FEATURES. ALONG ALL SUCH UNDERGROUND UTILITIES, A DIKE OF CLAY OR OTHER SUITABLE MATERIAL SHALL BE CONSTRUCTED ACROSS THE TRENCH OF THE TRANSMISSION LINES AND PIPELINES AT INTERVALS OF 20 FEET OR LESS, OR AS DIRECTED BY A PE OR PO. 8. FOR ANY TESTS REQUIRING BOREHOLES, SUCH AS AIR TRACK DRILLING, THE BOREHOLES MUST BE GROUTED UPON COMPLETION. GROUTING SHOULD BE DONE WITH A MIXTURE OF 50% BENTONITE AND 50% PORTLAND CEMENT. 9. IF AIR TRACK DRILLING IS USED TO DETERMINE THE DEPTH OF OVERBURDEN AND CONTINUITY OF BEDROCK, THEN THESE OPERATIONS MUST BE MONITORED FULL TIME BY A GEOTECHNICAL ENGINEER TO CONFIRM THE FINDINGS OF THE DRILLER. 10. STORAGE TANKS SHALL HAVE IMPERVIOUS SECONDARY CONTAINMENT. UNDERGROUND FUEL STORAGE TANKS SHALL HAVE INTERSTITIAL MONITORING OF TANKS AND PIPING SYSTEMS. 11. WHERE APPLICABLE, THE FOLLOWING CONSUMER DISCLOSURE STATEMENT THAT PROVIDES INFORMATION ON WHAT REVIEW OCCURRED AND WHAT WAS DISCOVERED SHALL BE INCLUDED IN THE DEED OF DEDICATION AND RECORD PLAT: THIS PROPERTY IS LOCATED IN AN AREA IDENTIFIED AS HAVING KARST FEATURES.
