1. The Authority's Water and SewerStandaMs and Specifications are available at www.fcsa-water.com
2. The contiectorsha/I adhere to the Authonty's standards and specifications in effect at the time of ronstrucdon.
3. The contractor shall coordinate with and arrange for inspection by the Authonty.
4. The contractor shall connect a new sewer line to an existing manhole by rare drilling the manhole.
5 A new water (or forced sewer) main shall be connected to an existing main by wet tap.
The Authority shall furnish all needed materia/ and make the tap.
An application for wet Tap shall be submitted and the Initial fee paid before tap is made.
6. Exact locations of water and sewer services on new lines are to be coordinated with the Authority's Inspector.
7 The Authority's maintenance di shat/ famish and install all water meters through 2"in size.
It is the contractor's responsibility to have the meter box asssmbty installed correctly.
Before a permanent meter is installed:
a) The meter box, with its frame and rover, must property aligned with the coppersetter.
b) The frame and cover shall be set to final grade.
c) The distance between the top of the cover and the coppersetter shall be between 18 and 21 inches.
d) All components of the meter box assembly shall be in proper working order.
8. For seraces that connect to existing lines:
a. The Authority shall fum/sh and install.,
1. all % Xa/+ , ala , 1, 11/a and 2" water services.
1. all sewer services.
b. The owner/developer shall.,
1. coordinate (or have the contractor coordinate) the location of the service lateral with the Authont" engineering assistant.
2. submit an Application for Service and pay the required fees.
9. All water service lines must have a backflow prevention assembly (double check valve or RPZ, as required). The assembly must meet
ASSEstandard number 1015 or 1013.
10. Air tire lines must have a badflow prevention assembly (detector double check valve ora RPZ, as required). The assembly shall meet
ASSE standard number 1048 or 1047 Any exterior, privately owned fire line shall also have a fire service meter. This assembly shall be
installed immediately before the backflow prevention unit. Radio read remotes are required.
11. The Authority shall review the mechanical p/an(s) for design and materia/ approval of building's:
a. domestic water meter and its backflow prevention device, and/or its
b. fire service line's backflow prevention device.
11. DEQ must also approve sewer pump stations. FCSA requires a copy ofDEQ's Certificate to Operate and a copy of the station's DEQ
approved O&M manual. These documents must be received before substantial completion is issued and water meters re/eased.

GENERAL NOTES
1. Permits must be issued from VDOT pnor to any construction in a public right-of-way.
2. The Manual on Uniform Trafl7c Control Devices (MUTC)) must be adhered to when working in a public right of way.
3. The contractor will remove all mud and debris from the public right of way that is telt by vehicles entering and leaving the site.
4. Any damage to neighboring properties induding county roads and sidewalks, caused by the development of this site will be the
responsibility ofthe developer. All damage must be repaired to ongina/ conditions.
5. A construction entrance must be installed in accordance with the "Virginia Erosion and Sediment Centro/ Handbook"Section 3.02. The
contractor must maintain this access for the duration of the project.
6. The contractor will be responsible for adequate and positive drainage ofthis site.
7. The contractor will obtain a Land Disturbance Permit prior to any site work or construction.
8. Al/ construction will be done following the laws and guidelines set forth by OSHA and PUSH.
9. The contractor will notify engineer of record of any discrepancies between the approved plans and existing site conditions.
10. The developer is responsible for the relocation of any utilities required by this site plan.
11. All Erosion and Sediment Control de vires shown on this site plan shall be installed prior to construction.

locations and not all utilities may be shown.
14. The contractor will contain all litter and debris on-site. The work must be left in an orderly manner at the end of each workday.
15. An approved set ofsite plans must be on-site during the construction process.
16. The contractor will notify all utility companies prior to excavation within easements.
17 The contractor will seed and straw, forsod/stabilization, any disturbed areas that will be left for more than thirty days.
DEPARTMENT OF INSPECTION GENERAL NOTES
1. Buildings shall comply with the Virginia Uniform Statewide Building Code & Sections 311, S (storage) ofthe International Building Code/2012. Other
codes that apply are ICC/ANSI All Z1-3-09 Accessible and the Usable Buildings & Fadlilles.
1. Proposed 10,875 sq it single story storage facility shall comply with Chapter 5 for height and area (7503). Limitations for type of construction in
Table 503 of the 2009 IBC and Fire Protection shall be insta/led as required in Chapter 918Cfor Group S-1, 903.2.9.
3. Building shall comply with ANSI Handicap A117.1-3-03 for parking, entrance and egress. All required egress shall be accessible.
4. Special Inspection requirements of Chapter 17 ofIBC shall apply to this structure and must be sea/ed by a Virginia Registered Design Professional.
(Soils, concrete, steel, etc), spedfical/y, fill fors/ab exceeding code shall require an engineered foundation.
5 A design professional licensed of the Commonwealth of Virginia shall seal the structural plans submitted for building permits.
FIRE MARSHALL GENERAL NOTES
1. An Inspection by the Fire Marshall's office will determine where curbing must be painted yellow.
1. 'No Parking - Fire Lane"signs will be placed on the site by the Fire Marshall's direction.
3. The contractor must align the fire hydrants to the diradlon as determined by the Fire Marshall
4. Buming brush piles ofnature/ vegetation requires a permit. Burning construction debris is prohibited.

VI. All work on this projectshall conform to the current editions and t revisions to the Virginia Department of Transportation (VDOT)
Road and Bridge Specifications and Standards, the Wrymia Buskin and Sediment Control Regulations, and any other applicable state,
federal, or lora/ regulations applicable. In rase of discrepancy or conflict between the Standards or Specifications and Regulations, the
most stringent shall govem.
VZ All construction shall comply with the latest U.S. Department of Lai vr, Occupational Safety & Health Administration (OSHA), and Virginia
Occupational Safety & Health (VOSH) Rules & Regulations.
V3. When working on VDOT right-of-way, all traffic control, whether permanent or temporary, shall be in accordance with the current edition
of VDOTs Work Area Protection Manual. A transportation management plan needs to be submitted for approval and land use permit
issued prior to any execution of work within the VDOT fight of way.
V4. The developer shall be responsible Por relocating, at his expense, any and all utilities, Including traffic signal poles, junction boxes,
controllers, etc, owned by VDOT or private/ public utility companies. It is the sole responsibility of the developer to locate and identify
utility facilities or items that may be in conflict with the proposed construction activity. MOOT approvat of these plans does not indemnify
the developer from this responsibility.
VS Design features relating to field construction, regulations, and control or safety of trefic may be subject to change as deemed
necessary by VDOT. Any additional expense incurred as a result of any field revision shall be the responsibility of the developer.
V6. If required by the local VOOTLand Development Office, a pre -construction conference shall be arranged and held by the engineer and/or
developer with the attendance of the contractor(s), various County agencies, utility companies and VDOT poor to initiation of work.
V7 The contractor shall notify the local VDOTLand Development Orrice when work is to begin or cease for any undetermined length of time.
VDOT requires 48 hours advance notice poor to any required or requested inspection.
V8. The contractor shall notify the Traffic Operations Center at (540) 331-9500 for any traffic control plan that impacts a VDO T maintained
Interstate or Primary madway to provide nodfiration of the installation and removal of the work zone.
V9. The contractor shall be responsible for maintaining a VDOT permit ed temporary construction entrance(s) in accordance with Section 3.02 of
the Virginia Erosion and Sediment Control Handbook Furthermore) access to other properties affected by this project shall be maintained
through construction. ,,
V10. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction.
Vll. A# water and sewer//nes within existing or proposed VDOTnght-o¢-way shall have a minimum thirty-six (36) inches coverand when
possible shall be installed under roadway drainage facilities at conflict points.
V12. Any unusual subsurface conditions (e.g., unsuitable soils, springs, sinkholes, voids, caves, etc.) encountered during the course of
construction shall be immediately brought to the attention of the engineer and VDOT. Work shall cease In that vicinity until an adequate
design can be determined by the engineerand approved by VDOT..
V13. All fill areas, borrow materia/ and undercut areas shall be inspected and approved by a VOOT representative prior to placement and All,
Where CBR testing is required, a MOT representative shall be present to insure the sample obtained ds representative of the location.
When $ol# samples are submitted to private laboratories for testing, the samples shall be dearly identified and labeled as belonging to a
project to be accepted by VDOT and that testing shall beperformed /n accordance with all applicable VDOTstandards and procedures.
V14. AI/ roadway 811, base, subgrede material, and backfill in utility/storm sewer trendies shall be compacted in six (6) inch lifts to 95% of
theoretical maximum density as determined byAASHTO T-99 Method A, within plus or minus 2% of optimum moisture for the full width of
any deoicated street right -tiff --way. At the direction of VDOT, densitytests shall be performed by a qualified independent agency in
accordance with VDOT Road and 8ndge Spedfiretions. Certified copies ofall test reports shall be submitted to VOOTdaily, unless specified
otherwise.
V15. VDOTStandard CD and UD underdrams shall be installed where indicated on these plans and/or as specified by VDOT.
V16. A post installation visual/video ramere inspection shall be conducted by the Contractor on all pipes identified on the plans as storm sewer
pipe and a select number ofp/pe culverts Far pipe culverts, a minimum of one pipe installation for each size of each material type will be
Inspected or ten percent of the total amount for each size and material type summarized All pipe installations on the plans not identified
as storm sewer pipe shall be considered as culvertpipe for inspection purposes. Additional testing maybe required as directed by the Area
Land Use Engineer or their representative.
V17. The installation of any entrances and mailboxes within any dedicated street fight -of --way shall meet VDOT minimum design standards and /s
the responsibility of the deve/oyer.
V18. Pnor to VOOTacceptance of any streets, all requiredstaetsignage and/or pavement markings shall be installed by the developer or, at
VDOTs discretion, by VDOT on an account receivable basis following the Manual On Uniform Traffic Control Devices.
V19. The developer must provide to the VDOT Land Development Office a list ofall materia/ sources prior to the start of construction. Copies of
all invoices for materials utilized within any dedicated street right -of --way must be provided to the local VOOT Residency Office prior to
acceptance of that work Unit and total prices may be obscured.
V20. Aggregate base and subbase materials shall be placed on subgrade by means of mechanical spreader. Density will be deterMned using
the density control step in accordance with Section 304 of the MOT Road and Bridge Specifications and VIM -10. A certified compaction
technician shall perform these tests. Certified copies of test reports shall be submitted to VDOT daily, unless specified otherwisein addition
to checking stone depths, a VDOT representative shall be notified and given the opportunity to he present during the construction and
testing ofthe density control strip.
V21. Asphalt concrete pavement shall be placed in accordance withSe on 315 of the MOT Road and Bridge Specifications. Density shall be
determined using the density contivl step as specified /n Section 15 and VTM-76. A certified compaction technician shall perform these
tests. Certified copies ofthe test reports shall be submitted to VDOT daily, unless specified otherwise. A VDOT representative shall be
notified and given the opportunity to be present during the construction and testing ofthe control strip.
V21. In accordance with Section 302.03, the foundations for pipe culverts thirty-six (36) inches and larger shall be explored below the bottom
ofthe excavation to determine the type and condition ofthe foun + tion. The contrectorsha/l report findings of foundation exploration to
the engineer and VOOT for approval prior to placing pipe. Founda n designs shall comply with VDOT Road and Bndge Standard PB -1.
Where soft, yielding, or otherwise unsuitable foundation is encomred, the foundation design and/or need for foundation stabilization shall
be determined by the engineer and approved by VDOT.
V23. VDOT Standard Guardrail shall be installed where warranted and. as proposed on these plans in accordance with VDOTs installation criteria.

V24. Approval ofthis plan shall expire five (5) years from the date of the approval letter
V25. VDOT Standard CG -11 Curb Ramps shall be installed where indicated on these plans and/or as specdied by MOT.
V26. The foundations for all box culverts shall be investigated by means of exploratory borings advanced below proposed foundation elevation to
determine the type and condition ofthe foundation. The contractorshall submit copies of boneha/e logs and report Wings offoundation
exploration to the engineerand VDOT forapprova/ prior to constructing box. Foundation designs shall comply with VDOT Road and Bridge
Standard PB -1. Contrary to the Standard, where rock is encountered and cast -In-place box is proposed, the thickness of bedding shall be
six (6) inches. Where sof, yielding, or otherwise unsuitable foundation is encountered, the foundation design and/or need for foundation
stabilization shall be determined by the engineerand approved by VDOT.
