A VDOT review has been conducted for the Lifegate Church Site Plan with a signature date of April
12, 2018. Based off of the review of the Drawings, all of the previous VDOT comments have been
addressed and the Site Plan is approved by our office. Please ensure that all applicable agencies
have the most recent set of approved Drawings.
We offer the following general comments:
• Our review and comments are general in nature. Should details be overlooked during plan
review or conditions in the field exist such that additional measures are warranted, such
measures shall be completed to the satisfaction of the department.
• Materials used and methods of construction shall adhere to the current observed VDOT Road
and Bridge Specifications, Road and Bridge Standards, Manua/ on Uniform Traifc Control
Devices, and Land Use Permit Special Provisions.
• A preconstruction conference shall be held by the engineer and/or the developer with the
attendance of the contractor, various County agencies, utility companies, and VDOT prior to
initiation of work.
All drainage is to be carried within the right-of-way in ditch lines or gutters along the street to
a pipe or drainage easement.
• Any construction related changes to the approved plan must come through the design
engineer to VDOT for approval. Please allow a minimum of 5 business days for VDOT review.
• A Land Use Permit shall be obtained before any work is performed on the State's right-of-
way. The permit is issued by this office and may require application fees, the salary &
expenses of a State assigned inspector, and surety bond coverage.

Dear Lifegate Free Will Baptist Church,
Your application for a conditional sewage disposal system permit filed with the Frederick County - City
of Winchester Health Department has been approved with the following conditions:
• Intermittent or seasonal use (this is ongoing). Intermittent usage of the facility as per the design
plans and specifications prepared by Racey Engineering. Peak, intermittent daily flow of 1250
gallons per event at no more than 2 events in any rolling seven day average. Maximum daily
dosing to the sewage dispersal field of 450 gallons/day.
Your permit is issued in accordance with the Code of Virginia, Title 32.1 and 32.1-164.1. Article I,
Board of Health, Commonwealth of Virginia, Sewage Handling and Disposal Regulations, and
current agency policy. This letter shall become and is a part of the sewage disposal system permit issued
for the above referenced location. Due to the conditional nature of this system, this permit shall not
become valid until the following items are completed.
1. The permit shall be recorded in the Grantor Index of the land records of the Circuit Court of the
County of Frederick.
2. The owner or his agent shall furnish to the Frederick County - City of Winchester Health
Department certification by the Clerk of the Circuit Court of the Deed Book Number and Page
Number upon which the permit and all provisions have been recorded. The Deed Book Number
and Page Number must be placed on all copies of the permit prior to giving validation to the
building officials for issuance of a building permit.
In accordance with the Sewage Handling and Disposal Regulations , you have the right of appeal to
obtain a modification or elimination of the conditions set herein. A written request to review the
conditions must be submitted to Colin Greene, MD, Director, Frederick County - City of Winchester
Health Department, 107 N Kent St. Ste 201, Winchester, VA 22601.
This Permit has been issued in accordance with applicable regulations based on the infortnation and
materials provided at the time of application. There may be other local, state, or federal laws or
regulations that apply to the proposed construction of this onsite sewage system. The owner is
responsible at all times for complying with all applicable local, state, and federal laws and regulations.

A VDOT review has been conducted on the Lifegate Church Site Plan with a
signature date of December 6, 2017. We offer the following comments:
1) We recommend the private street connection for Soldiers Rest Lane at Rte.
651, be hard surfaced for quicker and safer ingress/ egress to Soldiers
Rest Lane. Please see figure 4-8 on page F-106 of the current VDOT Road
Design Manual for private street connection dimensions.
2) There is currently no outfall for the drainage associated with this
development. Please evaluate the storm water for the 10 year and 25 year
event for impacts to the roadway.
Please provide a resolution for the above comments by sending two sets of revised
drawings, a comment response letter and highlighted plan showing changes, as well
as one digital copy of the plans. Should you have any questions, please feel free to
contact me.

sanitary sewer will be served by onsite well and sanitary drainfield systems.

Site is identified as TM 74-3-1, and is located at the intersection of Shady Elm Road (Rte. 651) and Soldiers Rest Lane.

Building shall comply with The Virginia Uniform Statewide Building Code, Use group A
(Assembly), The International Building Code / 2012. Other code that applies is
ICC/ANSI A 117.1-09 Usable and Accessible Buildings and Facilities and 2012
International Energy Code.
Building shall meet height and area of T503 with applicable increases for type of
construction. A-3 occupancies where the fire area is located on a floor other than the exit
discharge or total fire area exceeds 12000 sq fl (1s' and 2nd floor) shall be provided with a
sprinkler system. (Section 903.2.1.2).
A vestibule is required at main public entrance that enter into spaces larger than 3000 sq

5%. Accessible parking and unloading shall not exceed 2%. Accessible route to main

minimum of 8' of striped unloading area. Unloading area shall have a maximum 21D/o
slope. Van accessible signage is provided as required per USBC Sec 1106.8.
Freestanding signs and wall signs require a permit.
Please note the requirements in Chapter 17 of IBC for special inspection requirements on
the type of structure. (Soils, concrete etc..).
