The contractor shall furnish all materials, lobor and equipment and perform all work including restoration for the completed installation of the utility systems and all other improvements shown or implied as necessary for the completed focility, ready for use. The contractor shall be responsible for all construction stakeout.. All dimensions, devotions and locations shall be verified in the field by each contractor prior to beginning the work. The Engineer will approve all cut sheets. 4. Any unusual subsurface conditions encountered during the course of the work shall be immediately brought to the attention of the Engineer and /or the Owner. 5. Contractor to obtain, of his expense, all permits required of this work and shall faithfully adhere to the permit conditions imposed. 7. Cutsheets shall be submitted to the Engineer for approval prior to ordering structures and start of construction. All cutsheets should show existing ground elevations at centerline of utility, structure or rood, and offset hub elevotion. FIRE RESCUE 1, Burning of land clearing debris requires a permit from the Fire Marshal. Burning of construction debris on site is prohibited, 2. Access for emergency vehicles must be maintained at all times during construction. Temporary street signs and addressed must be posted as construction begins. Parked vehicles and staging of construction materials are prohibited within 20 of fire hydrants. Fire Hydrants are not to be obstructed at any time. FREDERICK COUNTY SANITATION AUTHORITY 1. Woter and sewer lines ore to be constructed in accordance with these plans. The lines shall additionally meet all requirements of the Frederick. County Sanitation Authority. 2. The contractor shall coordinate with, and arrange for inspection by the Authority. 3. Exact locations of water and sewer services on new lines are to be coordinoted with the Authority's inspector. 4. The Sanitation Authority's maintenance department is responsible for the instolla ion of water meters through 2 in size. Before d: permanent meter is set, they require; 1) the frame and cover to be properly aligned with its meter box and yoke bar, 2) the frame and cover to be set to the final grade of the lot; 3) the distance between the top of the cover and the yoke bar be between 18 and 21 inches; and 4) alh erhponents of the meter box assembly to be in proper working orderlt is developer sfcontractor s responsibility to have the meter box assembly installed correctly. 5. For water and sewer services that connect to existing lines: a. The location of the service lateral' is to be coordinated with the Authority's engineering. assistant. An application for service must ,be mode and Lees paid before work will begin. b. The Auth ra shag "furnish and install all 5/8 3/4 and 1 water services. c. For 1 1/2 and 2 water services, the Authority shall make the top and bring the piping to the property line. The contractor is to furnish and install the meter vault assembly.. The Authority will install the meter. d. The Authority will install all sewer Ioterats. 6. When an existing water main is tapped for the installation of a new line; a. The Authority hall furnish all labor and materials to make the top. b. The developer contractor must: (1) make application for the top, (2) pay deposit (before work begins; (3) mark the. location of the top; (4) make final poyment when the work is completed. 7. The contractor shall tie -in a new lie to an existing manhole b core drilling the manhole. 8. Any water service fine must have a double check valve or RPZ backftow prevention assembly, as required, installed. The assembly shall meet ASSE standard NO. 1015 or 1013. A remote readout is. required. 9. At fire lines shall hove a detector double check valve backftow prevention assembly or a RPZ backftow prevention assembly as required. The assembly shall meet ASSE standard No. 1048 or 1047. A remote readout is required. VIRGINIA DEPARTMENT OF TRANSPORTATION 1. All work on this project shall conform to 'the latest editions of the Virginia Deportment of Transportation (VDOT) Road and Bridge Specifications, and Standards, The Virginia Erosion and Sediment Control Regulations and any other state, federal or local regulations applicable. In the event of conflict between any of these standards, specifications or plans, the most stringent shall govern. 2. All construction sholl comply with the latest U.S. Department of Labor Occupational Safety Health Administration and •VOSH Rules Regulations. 3. When working on VDOT right-of-way, all traffic control, whether permanent or temporary shall be in accordance with the current edition of VDOT 'l work area protection manual. Furthermore, all traffic control flaggers must be certified in accordance with VDOT s January, 1994 (or latest edition of) Road Bridge Specifications, Section 104.04 -C. 4, The developer shall be responsible for relocatin at his expense, any and all utilities including traffic signal poles junction boxes, controllers, etc. owned by V.D.O.T. or private utility company. It is the sole responsibility of the developer to locate and identify utility facilities or items which may be in conflict with the proposed construction activity. V.D.O.T approval of these plans does not relieve the developer of this responsibility. 5.: Design features relating to construction or to regulation, control and safety of traffic may be subtect to changqe as deemed necessary by VDOT. Prior to initiation of work, Contractor shall be responsible for acquiring all necessary VDOT land use permits for. any work, on VDOT right -of -way. 7. If required by the Ideal VOOT Residency Office, a, preconstruction conference- must,be,:arrapged ond,beld- byn'the engineer and /or developer with the attendance of the controctor, various County agencies, utility companies and VDOT prior to initiation of work. 8. Contractor shall notify the local VDOT Residency Office when work is to begin or cease for any undetermined length of time. VDOT will also require 48 hours notice for any inspection. 9. The Contractor will be responsible for maintaining odequote access to the project from the adjacent public rood woy through construction and maintenance of a construction entrance in accordance with the Virginia Erosion Sediment Control Handbook, Sec 3.02. Furthermore, access to other properties affected by this project shall be maintained through construction. The Developer shall have, within the limits of the, project, an employee certified by the Department of Conservation and Recreation in Erosion and Sediment Control who shall inspect erosion and siltation control devices and measures for proper installation and deficiencies immedk'tely after each rainfall, at least daily during prolonged rainfall, and weekly when no rainfall event occurs. 10. Contractor shall ensure adequate drainage is achieved and maintained on the site during and at the end of construction. 11.. All 'water and sewer lines within existing or proposed VDOT right-of-way are to have minimum 36" cover and, when possible, to be installed under roadway drainage facilities at conflict points. 12. Any unusual subsurface conditions encountered during the course of construction shall be immediately brought to the attention of the engineer and VDOT. Work shall cease in that vicinity until an adequate design can be determined by the engineer and approved by V,D.O.T. 13. Ali fill areas, borrow material and undercut areas shall be inspected and approved by VDOT Inspector prior to placement of fill. Where CBR testing is required, a V.D.O.T representative shalt be present to ensure the sompte obtained is representative of the location. When soil samples are submitted to private tobratories for testing, the sar'npies shall be clearly identified and labeled os belonging to a project which will be accepted by V.D.O.T. and that testing shall be conducted according to applicable V.D.O.T. standards. 14. M roadway fill, base, subsurface material and backfill of utili storm sewer trenches shall be compacted in 6 AA lifts to 95% of theoretical maximum density os' determined by MSHTO T -99 Method A, within plus or minus 2% of optimum moisture for the full width of any dedicated street right-of-way. At the direction of VDOT Inspector density tests performed by a qualified independent agency shall be conducted as required in the VDOT Rood and Bridge Specifications. A copy of all tests shall be submitted to VDOT prior to final VDOT approval. 15. VDOT Standard CO and UD underdrains shall be installed where indicated on these plans and further where determined necessary by VDOT Inspector. 16. The installation of any entrances and mailboxes within any dedicated street right -of -way shall meet VDOT minimum design standards and is the developers responsibility, 17. Prior to acceptance by VDOT of any streets, any required street signage and /or pavement markings must be instalta by the developer or, at VDOT's discretion, by VDOT on an account receivable basis. Stop signs shall be thirty (30) inches, tall with high intensity sheeting. The bottom of the sign shall be six (6) feet above the edge of pavement and installed a m;;nimum of four (4) feet from the edge of pavement. Sign posts shall be of a type found on V.D.O.T.'s approved list. A copy, of this list may be obtained from the District Material Section. Contact' the local V.D.O.T. residency office with any questions regarding sign material and plocement. 18. The developer must provide to the local V.D.O.T: office a List of all material sources prior to construction. Copies of bII invoices for materials within any dedicated street right of way must be provided to the local V.D.O.T. residency office prior to acceptance of the work. Unita and total prices may be obscured. 19. Aggregate bases and sub -bases shall be placed on the sub grade by means of a mechanicol spreader. Density win be determined using the density control strip as specified in V.D.O,T.'s Road and Bridge Specifications (Section 304) and VTM -10, A certified compaction technician shall perform these tests. A certified copy of the test results shall be provided to the Residency Office. in addition to checking storn depths, a V.D.O.T. representative shall be notified and given the opportunity to be present duriig the construction and testing of the density control strip. 20. Asphalt concrete pavement shall be placed in accordance with the latest edition of V.D.O.T.'s Beet and Bridge Spcxifwations (Section 315), Density will be determined using density control strip as specified in Section 315 and VTM -76. A certified compaction technician shall perform these tests. A certified copy of the test results shall be provided to the Residency Office. in addition to checking storn depths, a V.D.O.T. representative shall be notified and given the opportunity to be present during the construction and testing of the density control strip. 21. Approval of these pions shall expire three (3) years from the date of the approval letter, 22. Temporary and permanent erosion control structures and methods to be in accordance with 'the Virginia Erosion .Control Guidelines and local ordinances and to be utiilized during the construction period 5. All erosion control structures shown or as required during construction shall be continuously 'maintained throughout the period in which areas are denuded. Areas shown to be surfaced are to have stone base installed at the earliest possible dote after establishing grade. No area is to remain denuded for more roan 7 calendar days when construction is not in progress. 6.
