Action:
BOARD OF SUPERVISORS March 27, 2024
RECOGNIZING APRIL 16, 2024, AS
NATIONAL HEALTHCARE DECISION DAY
WHEREAS, illness can strike suddenly and leave one unable to snake medical decisions for oneself, and
WHEREAS, an Advance Directive (Living Will) is deemed very important for all of our citizens; and
WHEREAS, if one is incapacitated and unable to make his or her own health decisions, an Advance
Directive allows for the appointment of a Medical Agent to act as a Surrogate Decision Maker and such a
document has been found extremely useful in ensuring the implementation of one's healthcare wishes when
unable to make thern for oneself; and
WHEREAS,April 16 has been highlighted as a day to ensure everyone over the age of 18 has an Advance
Directive; and
WHEREAS, Valley Health is offering a free face-to-face interview with an experienced healthcare
professional to aid in filling out Advance Directive documents, and the interview does not require a
physician referral and can be scheduled by calling 540-536-5437; and
WHEREAS, Valley Health is offering a flee speakers bureau to provide educational speakers to any
organization on the importance of an Advance Directive.
NOW, THEREFORE, BE IT RESOLVED that the Frederick County Board of Supervisors recognizes
April 16 as NATIONAL HEALTHCARE DECISION DAY and strongly endorses the use of an Advance
Directive by the citizens of Frederick County.

SUBJECT: Resolution Recognizing National Healthcare Decision Day

April 16th has been highlighted as a day to ensure that everyone over the age of eighteen has
an Advance Directive. An Advance Directive (living will) is a written, legal instruction
regarding your preferences for medical care if you are unable to make decisions for yourself.
Advance directives guide choices for doctors and caregivers if you are terminally ill, seriously
injured, in a coma, in the late stages of dementia or near the end of life.
The attached resolution recognizes April 16th as National Healthcare Decision Day and
endorses the use of an Advance Directive by the citizens of Frederick County.
