RESOLUTION
LONG-TERM CARE COORDINATING COMMITTEE FOR FREDERICK COUNTY
DESIGNATION OF LEAD AGENCY
WHEREAS, Section 2.2-708 of the Code of Virginia requires that the governing body of
each county or city designate a lead agency and member agencies to accomplish the coordination
of local long-term care services; and
WHEREAS, in 2002, the Frederick County Department of Social Services collaborated with
the Community Services Board to establish a committee to coordinate a local response to the
provision of services to elderly and disabled individuals over the age of 18 in our area and, for the
past four years, said committee has been meeting monthly at the Department of Social Services; and
WHEREAS, as designated in Section 2.2-708, the membership ofthis on-going committee
includes representatives of the following agencies:
• Frederick, Winchester and Clarke Departments of Social Services
• Shenandoah Area Agency on Aging
• Winchester Medical Center (both the ER and Adult Psychiatric Units)
• Vocational Rehabilitation Services
• Northwestern Community Services Board
• Law Enforcement
• Salvation Army
• Winchester-Frederick County Health Department; and
WHEREAS, the provision of long-term care includes services to disabled individuals over
the age of 18 as well as services to elderly disabled individuals;
THEREFORE, BE IT RESOLVED, that the Board of Supervisors designates the Frederick
County Department of Social Services to serve as the lead agency for the coordination oflong-teen
care in Frederick County.
ADOPTED, this 24`h day of January, 2007.

History of Resolution for Designation of Lead Agency for the Long-
Term Care Coordinating Committee for Frederick County:
On October 6, 2006, AARP Virginia sent a communication to all City Managers and County
Administrators in Virginia requesting information on the designated lead agency for long-
term care for the locality. In their cover letter requesting this information, the AARP cited a
law (which has been on the books since 1982) which requires each political jurisdiction "to
designate a lead agency and member agencies to accomplish the coordination of local
long-term care services."
On October 12, John Riley sent out an a-mail to Gwen Monroe, Director of Social Services,
and to Dr. Diane Helentjaris, Director of the Winchester-Frederick County Health
Department, with a copy of the correspondence from the AARP Virginia, asking if either of
them were familiar with this program and requesting that the two of them take a leadership
role in implementing same. Gwen Monroe responded, with a copy to Dr. Helentjaris and
Helen Cockrell, that for the past four years, the Frederick County Department of Social
Services has been actively involved in coordinating a committee which includes the same
agencies designated by law for the Long-Term Care Coordinating Council. This committee
meets monthly at the Frederick County Department of Social Services and coordinates
services for the elderly and for disabled individuals over the age of 18. Ms. Monroe offered
that the Department of Social Services was willing to serve as the lead agency for the
coordination of long-term care for Frederick County. Dr. Helentjaris responded with a letter
of support for the Department of Social Services to serve as lead agency and arranged for a
health department representative to become actively involved in the on-going monthly
meetings.
Helen Cockrell, Director of the Shenandoah Area on Aging, wrote to all County
Administrators and to the City Manager in the Lord Fairfax Planning District also offering a
willingness for the Shenandoah Area Agency on Aging to be designated as the lead agency
for coordinating long-term care services in Planning District 7.
The attached resolution for the designation of the Frederick County Department of Social
Services to serve as the lead agency for Frederick County takes into account the already
established adult services coordinating meetings which have been on-going since 2002 and
consideration that long-term care needs include services to disabled individuals between
the ages of 18 and 59, as well as services to individuals over the age of 60 served by Area
Agencies on Aging.

Thank you for volunteering the services of the Shenandoah Area Agency on Aging
regarding the coordination of long term care for Frederick County. At its regularly
scheduled meeting on January _, 2007, the Frederick County Board of Supervisors
designated the Frederick County Department of Social Services as the jurisdiction's lead
agency of the coordination of the long term care. Consideration was given to the
provision of services to disabled adults between the ages of 18 and 59 (in addition to the
disabled adults over the age of 60) as well as the already established adult services
- coordinating meetings which have been on-going since 2002. These monthly meetings
currently include representation from all of the agencies designated in the Code of
Vir inia, Section 2.2-708.
We appreciate the involvement of the Shenandoah Area Agency on Aging in the on-
going coordinating meetings and appreciate your expressed willingness to assume a
leadership role in long term care coordination for Frederick County. We look forward to
your continued collaboration with the Department of Social Services as we work towards
the provision of long- term care services for Frederick County residents.

Subject: Coordination of Local Long-Term Care Services in Virginia
A:ARP Virginia began a campaign in 2005 to improve Virginia's long-term care delivery system to meet
not only current needs but those of an aging population. The Virginia Gareral Assembly Joint Legislative
Audit and Review Commission in its 2006 report, Impact of an Aeinn Pooutation on State A¢arcies,
projected that there will be 1.3 million more older Virginians by 2030 than there were in 2000 - a 120%
increase. Because such a population increase obviously will need more services than arc currently
available, coordination and cooperation of delivery of services of the local level is most important.
Section 2.2-708 - Coordination of local long-term care services (see below) requires "the governing body
of each county or city, or a combination thereof, shall designate a lead agency and member agencies to
accomplish the coordination of local long-term care services."
§ 2.2-708. Coordination of local tong-term care services.
The governing body of each county or city, or a combination thereof, shall designate a lead
agency and member agencies to accomplish the coordination of local long-term care services. The
agencies shall establish along-term care coordination committee composed of, but not limited to,
representatives of each agency. The coordination committee shall guide the coordination and
administration of public long-term care services in the locality. The membership of the
coordination committee shall be comprised of, but not limited to, representatives of the local
department of public health, the local department of social services, the community services
boazd or community mental health clinic, the azea agency on aging and the local nursing home
pre-admission screening team. A plan shall be implemented that ensures the cost-effective
utilization of all funds available for long-term care services in the locality. Localities are
encouraged to provide services within each category of service in the continuum and to allow one
person to deliver multiple services, when possible. (1982, c. 346, § 2.1-373.7; 2001, c. 844.)
